An audible and visual alarm system to give a warning on both sides of the airlock should be provided to indicate if more than one door is moved from the closed position. PSCO observed audible alarm not sounding while both airlock doors were open at the same time. Outer alarm is inoperable and needs repair.

Incinerator exhaust pipe damper malfunctioned resulting in HI-FOG system activation. HI-FOG nitrogen cylinders are below required pressure.
