Vessel did not have a suitable first aid kit onboard, replace with new one or update previous contents with required items. 46 CFR 184.710

Vessel does not have FCC certification for intended route. Vessel is limited to 1000 ft from shore. 46 CFR 184.502

Observed 08 feet of 2" aluminum angle iron missing from the rub rail on port side aft pontoon. Replace angle iron in accordance with original drawings. 46 CFR 177.202

Observed broken welds on port and starboard pontoons where the rub rail is attached. Repair welds in accordance with original drawings. 46 CFR 177.202

Observed splash guards damaged on port and starboard pontoons. Re-weld port side, fabricate and replace 18 inch section for starboard side and re-weld in accordance with original drawings. 46 CFR 177.202

Observed missing hose clamps on port engine fuel hoses, replace hose clamps to meet double hose clamp requirement. 46 CFR 182.455

Observed 02 benches, forward fixed seating, loose with missing hardware. Replace hardware and ensure seats are secured in place. 46 CFR 177.800

Child size life jackets equal to at least 10% of number of persons required. 1 child life jacket non-operational. Replace child life jacket. Vessel limited to 90 passengers.

A vessel on a rivers routes must carry three hand red flares. Vessel did not have 3 red flares. Provide 3 red flares. Vessel limited to daylight operations.

The name of a vessel must be marked or painted in clearly legible letter. Life buoy was missing letter. Replaced letters.

Portable extinguishers must be inspected and maintained in accordance with NFPA 10. Fire extinguisher not serviced. Replaced with new fire extinguisher.

Portable fire extinguisher installation must be to the satisfaction of the Officer in Charge, Marine Inspection. Portable fire extinguisher brackets were rusted/inoperable and require replacement.

Shutoff valves, installed so as to close against the fuel flow, must be fitted in the fuel supply lines. If the handle to the shutoff valve at the tank is located inside the machinery space, it must be located so that the operator does not have to reach more than 12 inches into the machinery space and the valve handle must be shielded from flames by the same material the hull is constructed of, or some noncombustible material. Fuel shutoff valve is more than 12 inches into space; valve required to be relocated outside of space.

A small passenger vessel which carries more than six passengers including at least one for hire may not be operated without a valid COI. Vessel missed its required annual inspection rendering the COI invalid. Vessel is required to complete a COI inspection prior to the carriage of more than six passengers.

A vessel on a lakes, bays, and sounds, or rivers route must carry: (1) Three hand red flare distress signals approved in accordance with § 160.021 in subchapter Q of this chapter, or other standard specified by the Commandant; and (2) Three hand orange smoke distress signals approved in accordance with § 160.037 in subchapter Q of this chapter, or other standard specified by the Commandant. Distress flares found to be expired.

A vessel must carry one floating waterlight, unless it is limited to daytime operation, in which case no floating waterlight is required. Waterlight battery was expired.

A vessel must carry either a first-aid kit that meets the requirements in 46 CFR 199.175(b)(10) or a kit with equivalent contents and instructions. Found multiple items to be expired.

Electrical equipment on a vessel must be installed and maintained to: (b) Protect passengers, crew, other persons, and the vessel from electrical hazards, including fire, caused by or originating in electrical equipment, and electrical shock. Positive cable on battery terminal found to be frayed and exposed.

Each vessel must be equipped with at least two operable portable battery lights. One of these lights must be located at the operating station and the other at the access to the propulsion machinery space. Vessel only equipped with one portable light.

Portable fire extinguishers must be located so that they are clearly visible and readily accessible from the space being protected. The installation and location must be to the satisfaction of the Officer in Charge of Marine Inspection. Forward fire extinguisher bracket was incorrect type for size of fire extinguisher.

At each inspection for certification and at every other vessel inspection all observed unsafe practices, fire hazards, and other hazardous situations must be corrected and all required guards and protective devices must be in satisfactory condition. Found numerous paint cans and other flammable liquids on the deck and not stored in an appropriate flammable liquids locker.

Cable and wire for power and lighting circuits must: (1) Meet Section 310-13 of NFPA 70 (incorporated by reference; see 46 CFR 175.600) except that asbestos insulated cable and dry location cables may not be used; (2) Be listed by Underwriters Laboratories (UL), as UL Boat or UL Marine cable; or (3) Meet § 111.60-1 in subchapter J of this chapter for cable, and § 111.60-11 in subchapter J of this chapter for wire. Found several unapproved extension cords in use throughout the vessel.

Repairs or alterations to the hull, machinery, or equipment that affect the safety of the vessel must not be made without the approval of the cognizant OCMI, except during an emergency. When repairs are made during an emergency, the owner, managing operator, or master shall notify the OCMI as soon as practicable after such repairs or alternations are made. Found unapproved repair on the port side pontoon transom. Provide NDT report for repair, or dry dock vessel and make permanent repair to satisfaction of the OCMI.

A VESSEL IS INSPECTED FOR COMPLIANCE WITH THE STANDARDS REQUIRED BY THIS SUBCHAPTER. FOUND TWO INCH CRACK ON WELD ON PORT AFT PONTOON. REPAIR WELD AND TEST PONTOON.

A seat must be constructed to minimize the possibility of injury. The fourth centerline bench is loose. Secure bench to the deck.

The master and crew of a vessel will be familiar with the content of and have mounted at the operating station, emergency instructions containing the actions to be taken in the event of fire, heavy weather, or man overboard conditions. No instructions for heavy weather posted; post heavy weather instructions.

An EAP training program must be conducted for the employer's crewmembers and supervisory personnel. Supervisory personnel must receive at least 60 minutes of training. Supervisory personnel do not have proof of training completion; conduct and document proper training.

The Certificate of Inspection must be posted under glass or other suitable transparent material, such that all pages are visible, in a conspicuous place on the vessel where observation by passengers is likely. If posting is impracticable, such as in an open boat, the certificates must be kept on board. Amended COI is not on board; obtain and post updated COI.

The person in command of a documented vessel must produce the original Certificate of Documentation currently in effect for that vessel upon the demand of any person acting in an official public capacity. Original COD not on board; obtain original.

The cognizant OCMI determines the total number of persons permitted to be carried on a vessel considering, among other things, stability restrictions and the maximum number of passengers permitted in accordance with 46 CFR 176.113. The Marine Safety Center issued a stability letter (Serial No. H1-1902614) to the vessel on 08/08/2019 which, when combined with the passenger criteria of 46 CFR 176.113, restricts the vessel to carry not more than 100 passengers. The maximum number of passengers permitted is therefore reduced from 106 to 100, with a maximum of 102 persons allowed on board, until the Certificate of Inspection is amended to reflect the current stability restrictions.

A vessel must carry an approved first aid kit under 160.041 or equivalent. Several first aid supplies are missing. Provide all required supplies.

The hull has two Hull ID Numbers affixed. Remove the incorrect Hull ID Number.

Properly mount the bull horn at the operating station.

Properly mount the Certificate of Inspection in a location visible to passengers.
