Thank you for the detailed update and for the prompt actions taken by your staff to remediate the mildew issue in the crew berthing. The Coast Guard takes crew habitability and sanitation very seriously. While we appreciate the thorough response—including the mattress replacements and HVAC corrections—we must ensure the space is fully safe for occupancy. Consequently, we are issuing a CG-835 to the AMERICAN HARMONY requiring a qualified professional (e.g., an Industrial Hygienist) to officially test and verify that the crew quarters are indeed sanitary.

Vessels described by 46 CFR 114.110(f) must submit plans to the cognizant OCMI, in accordance with 46 CFR 115.700, for the installation and use of monitoring device(s) to ensure the wakefulness of the watchmen required in paragraph (a) of this section. Vessel's FIRE WATCHKEEPING PLAN, Revision 1, dated January 30, 2023, states that: "The Security Watchstander must carry a Motorola handheld radio with Lone Worker-Emergency function programmed, see Encl (1) for specifications." Vessel did not have the radio onboard as per their required plan. Obtain proper radio as per plan and provide proof to the satisfaction of the MI.

Except as otherwise provided in this section, each space accessible to passengers or used by the crew on a regular basis, must have at least two means of escape, one of which must not be a watertight door. The partitions between passengers state rooms on the outside decks need to be of a breakaway style to allow for egress and access to the fire dampers. Partitions had stainless steel bolts securing them in place and would not be easily opened in an emergency. Replace with breakaway style devices.

At each inspection for certification and at every other vessel inspection all observed unsafe practices, fire hazards, and other hazardous situations must be corrected and all required guards and protective devices must be in satisfactory condition. Grease was observed leaking from the galley hood extraction fan and was dripping onto the ladder, handrails and deck of the aft machinery space. Vessel shall clean and replace any leaking gaskets and seals to prevent any further leaking of grease.

The 3 panels on the bridge were missing the middle screws, making them not totally enclosed, creating a potential for shock and electrical hazards. Each vessel must be equipped with at least two operable portable battery lights. One of these lights must be located at the operating station and the other at the access to the propulsion machinery space. Upon testing the lights, neither of them worked. Vessel replaced batteries on the spot and proved proper operation.

ALL OBSERVED UNSAFE PRACTICES, FIRE HAZARDS, AND OTHER HAZARDOUS SITUATIONS MUST BE CORRECTED. ON 28JUL23, THE AMERICAN HARMONY EXPERIENCED A SMALL FIRE IN THE BACK PORCH GALLEY ON THE 4TH DECK CAUSING DAMAGE TO ELECTRICAL CORDS AND AN OUTLET.
THE VESSEL HAS PERMISSION TO PROCEED TO THE DALLES ON 29JUL23 WITH THE FOLLOWING REQUIREMENTS:
-AN ELECTRICIAN AND ENGINEER WILL MEET THE VESSEL TO EXAMINE THE ELECTRICAL SYSTEM AND DETERMINE THE CAUSE OF THE FIRE.
-THE ELECTRICAL WILL REMAIN SECURED IN THAT SPACE UNTIL IT IS DEEMED SAFE TO BE ENERGIZED.
-SET ADDITIONAL FIRE WATCHES IN THAT SPACE WHILE PASSENGERS ARE ON BOARD UNTIL IT IS DEEMED SAFE TO RETURN TO SERVICE.
-REVIEW PORTABLE FIRE EXTINGUISHER QUANTITY ON COI AND REPLACE EXTINGUISHER IF NECESSARY.
-MARINE INSPECTOR WILL MEET THE VESSEL IN THE DALLES AT 1800 ON 29JUL23 WHEN THE VESSEL ARRIVES.

A keel cooler installation used for engine cooling must be designed to prevent flooding. Water ingress was found due to a leak in the mounting frame gasket on the port side keel cooler. Until permanent repairs are made: 1) complete temporary repair with epoxy and shoring such that ingress of water is completely stopped, and 2) include monitoring of affected area as part of regular watchstander rounds.

At the last annual inspection the rescue boat was found to have insufficient stability to safely recover a MOB over the gunwale.
The [name withheld], Or issued a CG 835 to replace the rescue boat within 60 days. The CG 835 was later cleared with American Cruise Lines assurance that the rescue boat would be replaced during the off season layup period.
The rescue boat has not been replaced. Replace the rescue boat with the agreed upon more stable boat prior to sailing from the Port of Astoria.
