Fire extinguishing equipment installed on a vessel in excess of the requirements of §§ 181.400 and 181.500 must be designed, constructed, installed, and maintained in accordance with a recognized industry standard acceptable to the Commandant (CG-ENG-4). MI observed 02 inoperable smoke detectors in aft accommodation area and port crew space. Repair smoke detection within 14 days. 46 CFR 181.120

Electrical equipment on a vessel must be installed and maintained to: Provide services necessary for safety under normal and emergency conditions. MI identified raw water leak on generator. Repair prior to carriage of passengers. 46 CFR 183.200(a)

Rails or equivalent protection must be installed near the periphery of all decks of a vessel accessible to passengers or crew. Equivalent protection may include lifelines, wire rope, chains, and bulwarks, which provide strength and support equivalent to fixed rails. MI identified unsatisfactory railing adjacent to the operating station on the stbd side and damaged chain links adjacent to the operating station on the port side. Repair prior to the carriage of passengers. 46 CFR 177.900(a)

Licenses for ship stations in the maritime services will normally be issued for a term of ten years from the date of original issuance, or renewal. FCC Radio Station Authorization expired 03JUN2024. Replace within 14 days. 47 CFR 80.25(a)

A vessel not required to have a power driven fire pump by § 181.300 must have at least three 9.5 liter (2 1/2 gallon) buckets, with an attached lanyard satisfactory to the cognizant OCMI, placed so as to be easily available during an emergency. MI identified 01 (2 1/2 gallon) fire bucket missing. Replace within 14 days. 46 CFR 181.610

At each initial and subsequent inspection for certification of a vessel, the owner or managing operator shall be prepared to conduct tests and have the vessel ready for inspections of the hull structure and its appurtenances, including the following: Inspection and operation of all watertight closures in the hull, decks, and bulkheads. Identified hole in the stern of the vessel near the camera. Repair within 14 days. 46 CFR 176.802(a)(2)

In systems and applications where flexible hoses are permitted to be clamped: Double hose clamping is required where practicable. Replace the hose clamps identified in port side engine room and secure the surrounding wires. Replace within 14 days. 46 CFR 182.410(d)(1)

All engines must be water cooled and meet the requirements of this paragraph. The engine head, block, and exhaust manifold must be water-jacketed and cooled by water from a pump that operates whenever the engine is operating. Raw water pump in engine room needs to be mounted properly. Repair within 14 days. 46 CFR 182.420(a)(1)

A Certificate of Inspection is valid for 5 years for all other vessels. Vessel's COI has expired - vessel must obtain a new COI prior to the carriage of passengers.

Gages to indicate engine cooling water discharge temperature and lubricating oil pressure shall be provided for all propulsion engines and located so as to be in view of the operator. Gages on the Fly Bridge helm station are obscured by grime making them unreadable to the master of the vessel. 46 CFR 182.20-5(b) (Old T)

A vessel of 12 meters (39 feet) or more in length shall be provided with a whistle... The whistle... shall comply with the specifications in Annex III to these Rules (33 CFR part 86). Ship's whistle does not function. 33 CFR 83.33(a)

All vessels shall be fitted with such ground tackle and hawsers... depending upon the size of the vessel, the waters on which it operates, and other operating hazards. Starboard, forward cleat has damage. Operator to provide repair plan to USCG, prior to alterations, for approval. 46 CFR 184.10-1(a) (Old T)

A vessel must carry either a first-aid kit that meets the requirements in 46 CFR 199.175(b)(10) or a kit with equivalent contents and instructions. Vessel's first-aid kits are expired. 46 CFR 184.710 (New T)

At each initial and subsequent inspection for certification, the marine inspector may require that an... man overboard drill be held under simulated emergency conditions specified by the inspector. Vessel was unable to conduct MOB drill. 46 CFR 176.808(g) (New T)

An existing vessel with a hull, or a machinery space boundary bulkhead or deck, composed of wood or fiber reinforced plastic, or sheathed on the interior in fiber reinforced plastic, must comply with the requirements of § 181.400 of this part on or before March 11, 1999. The following spaces must be equipped with a fixed gas fire extinguishing system, in compliance with § 181.410, or other fixed fire extinguishing system specifically approved by the Commandant:(1) A space containing propulsion machinery. Vessel fixed firefighting system is not in compliance with 181.410. Vessels current pressure switch does not secure the engines as required by 46 CFR 181.410(b)(10). Furthermore, the vessels pressure switch is located in the space which is prohibited by 46 CFR 181.410(b)(1)(i)(ii). Vessel must submit plans to bring the installed fixed firefighting system in full compliance with 181.410. Your vessel must undergo an annual inspection with the 3 months before or after each anniversary date. Vessel inspection was canceled after Coast Guard Marine Inspectors arrived to conduct inspections. Complete an annual inspection prior to carrying passengers. 46CFR176.500(b)(1)

It is further intended that all machinery be installed and maintained in such a manner as to afford adequate protection against fire, explosion, machinery failure, and personnel injury. Inspectors observed soft patches on both fuel pipelines, and witnessed slight leaking while the MDEs were running. Vessel must either: replace damaged piping to original, or replace with proper non-metallic flex hose and fittings, as authorized in 46 CFR 182.20-40.

Vessel had expired Random Chemcial Testing Program Certificate of Enrollment. Supply updated random test results of crew.
