Before the ship leaves port all life saving appliances shall be in working order and ready for immediate use. Shackles on the forward embarkation ladder are seized, making the embakation ladder not ready for immediate use. There is no evidence of routine maintenance.

The last 1 meter of vapor piping upstream of the vessel vapor connection and each end of a vapor line must be labled with the word "VAPOR" painted in black letters. The stenciled word "VAPOR" was located approximately 3 meters from the connection point.
