Means shall be provided to control leaks of flammable liquids. PSCO observed pooled oil around the middle deck of the main engine, leaks throughout the main engine, leaks being blocked by oily rags and caught in unauthorized buckets and bottles surrounding the main engine. The No. 2 HFO purifier and No. 2 lube oil purifier were leaking as well as the No. 3 generator. Oil was also observed leaking from the valves on the LSDO and MDO tanks and the fuel oil pipeline.

Action required: 17 - Rectify deficiencies prior to departure
