At least two widely separated and ready means of escape shall be provided. PSCO observed door, starboard side main deck, unable to open from the inside.

74 SOLAS (14 CONS) II-2 4.1.1 Means shall be provided to control leaks of flammable liquids. PSCO observed fuel leaks coming from the main engine and generators. Buckets and rags were placed in various areas around the engine room to contain leaking flammable liquids. Emergency generator fuel tank containment had pooled oil.

74SOLAS(04CONS) II-1 13.2 Steering gear compartment arrangements shall include non-slip surfaces to ensure suitable working conditions in the event of hydraulic fuel leakage. PSCO observed oil soaked rag around pipe on hydraulic oil line and on deck where there were no non-slip surfaces.

74SOLAS(14CONS) Doors required to be self-closing shall not be fitted with hold back hooks. Doors leading into the scullery and galley were fitted with hold back hooks and when disconnected the doors could not self close as per the fire control plan.

Resolution: Class and CG attended vessel and determined that doors are not required to be self closing. Fire plan must be redrawn to update fire door details.
