The substandard conditions identified during each of the three detentions gives reasonable cause to believe that S&B Blessing LLC. and the vessel’s senior officers have failed to effectively and systematically implement the Safety Management System as required by the International Safety Management Code. See documents for Letter of Denial.

The means for securing and maintaining weathertightness shall be to the satisfaction of the Administration. The arrangements shall ensure that the tightness can be maintained in any sea conditions... PSCO’s observed approximately 05 holes on the cargo hatch covers ranging in size from 2” to 3”, a significant amount of corrosion, and damaged seals rendering the cargo hatch covers to not be weathertight and potentially compromised during adverse sea conditions. ICLL 66, Reg 16(4)

The company and the ship shall comply with the requirements of the International Safety Management Code. For the purpose of this regulation, the requirements of the Code shall be treated as mandatory. The company should establish procedures to ensure that the ship is maintained in conformity with the provisions of the relevant rules and regulations and with any additional requirements which may be established by the company. In meeting these requirements, the company should ensure that any non-conformity is reported, with its possible cause, if known. The nature of the below deficiencies provides objective evidence of a serious failure of the implementation of the ISM Code. An external ISM audit report, satisfactory to the Coast Guard, must be submitted prior to clearance of this deficiency and departure. 74 SOLAS (20) IX/3.1 ISM Code Part A/10.1

All fire protection systems and appliances shall at all times be in good order... PSCO’s observed a leak in the main fire pump case while both in the on and off position. PSCO’s also observed the fire main pipe located on the forward port side was missing nuts at the pipe connection point. CCSSC CH.3 / REG. 3.6.12

Hydrostatic release unit, other than disposable units shall be serviced: at intervals not exceeding twelve months; however in cases where it appears proper and reasonable, the administration may extend this period up to a maximum of seventeen months. PSCO observed the hydrostatic release unit on the EPIRB was expired as of September 2023 and had not been serviced in 12 months. CCSSC Ch. 4 / Reg. 4.8.1

All vessels shall carry up-to-date charts, appropriate for the intended voyage, of a large enough scale to enable safe navigation. PSCO observed chart #11460 had no chart corrections and was not kept up-to-date in accordance with Notice To Mariners chart updates. CCSSC CH 6 / Reg 6.3.1

Life-saving appliances required by this chapter shall comply with the technical specifications of the LSA Code and National Regulations as appropriate. The normal equipment of every rescue boat shall consist of: a first-aid outfit in a waterproof case capable of being closed tightly after use. PSCO’s observed the first-aid kit for the rescue boat was expired as of 31August2023. CCSSC CH 4/ Reg 4.3.1 LSA Code 5.1.2.2.9

The company and the ship shall comply with the requirements of the International Safety Management Code. For the purpose of this regulation, the requirements of the Code shall be treated as mandatory. The company should ensure that all personnel involved in the company's safety management system have an adequate understanding of relevant rules, regulations, codes and guidelines. The nature of the below deficiencies provides objective evidence of a serious failure of the implementation of the ISM Code see (form b). An external ISM audit report, satisfactory to the Coast Guard, must be submitted prior to clearance of this deficiency and departure. 74 SOLAS (20) IX/3.1 ISM Code Part A/6.4

Ships shall comply with the following requirements of SOLAS Chapter V…No blind sector, caused by cargo, cargo gear or other obstructions outside of the wheelhouse forward of the beam which obstructs the view of the sea surface as seen from the conning position. The PSCOs observed loading of cargo stowed and secured in a way that the ship and persons on board are put into risk. PSCOs observed limited bridge visibility from the conning position. CCSSC CH 6.5 74 SOLAS (20) V Reg 22.1.2

Any oil or other substance flammable or harmful to the marine environment shall not be carried in other tanks or spaces which are not specifically approved by the Administration for such purposes. PSCOs observed multiple cooking oil containers being shipped as cargo which were unsecured which pose a risk to the environment. CCSSC CH 3.21

Stairways and ladders shall be arranged to provide, from accommodation spaces, service spaces, control spaces, machinery spaces, and other spaces in which the crew is normally employed, ready means of escape to the open deck and thence to the survival craft. PSCOs observed boxes of debris and miscellaneous items in the accommodation space limiting escape in the event of an emergency. CCSSC CH 3.17.4.1

At position 1 and 2 the height above the deck of hatchway coamings fitted with weathertight hatch covers of steel...fitted with gaskets and clamping devices. PSCOs observed fire damper behind galley with no rubber gasket on one side. ICLL 66 Annex 1/Reg. 16.1

A fireman's outfit shall consist of personal equipment comprising protective clothing of material to protect the skin from the head radiating from the fire. PSCOs observed exposed skin from the firefighter's face due to no flash hood. CCSSC 3.11.2.1.1

Surfaces of all decks shall be so prepared or treated as to minimize the possibility of persons slipping. In particular...areas at the foot and head of ladders. PSCOs observed ladder on starboard side is damaged at 03 points. CCSSC 2.12.5

The company and the ship shall comply with the requirements of the International Safety Management Code. For the purpose of this regulation, the requirements of the Code shall be treated as mandatory. The company should establish procedures to ensure that the ship is maintained in conformity with the provisions of the relevant rules and regulations and with any additional requirements which may be established by the company. In meeting these requirements, the company should ensure that any nonconformity is reported, with its possible cause, if known. PSCO’s observed multiple smoke detectors with batteries disconnected, excessive rust in the fire main, fuel oil leaks from the main diesel engine, holes in fire boundaries, wasted fire main flange bolts, missing fire main valve steam hardware and excessive corrosion throughout the vessel. An ISM audit is required prior to departure. 74SOLAS (2020) IX/3.1 ISM Code Part A

The construction and the means for securing the weathertightness of cargo and other hatchways in positions 1 and 2 shall be at least equivalent to the requirements of regulation 16 and 15... PSCOs observed multiple areas of excessive corrosion and wastage on the cargo hatch covers and on cargo hold sides, compromising their structural strength and weathertight integrity. Upon expanding the exam, PSCOs observed light through the hatch covers in every cargo hold. ICLL 66 Annex 1/Reg 14.1

At position 1 and 2 the height above the deck of hatchway coamings fitted with weathertight hatch covers of steel or other equivalent material fitted with gaskets and clamping devices shall be as specified in regulation 15(1)… PSCOs observed wastage on knife edges on engine room ventilation, impeding the cover from sealing properly. One knife edge was cut to allow an electrical shore tie cable to go through the cover. ICLL 66 Annex 1/Reg. 16.1

Where air pipes to ballast and other tanks extend above the freeboard or superstructure decks, the exposed parts of the pipes shall be of substantial construction.... PSCO’s observed ballast water vent pipes throughout the vessel are excessively corroded, revealing holes in the pipes. ICLL 66 Annex 1/Reg 20

All machinery and electrical installations, mechanical and electrical equipment... shall be of a design and construction adequate for the service for which they are intended and shall be so installed and protected as to reduce to a minimum any danger to persons on board, due regard being paid to moving parts, hot surfaces and other hazards. PSCO’s observed multiple fuel leaks on main diesel engine and engine fuel pumps. Additionally, PSCOs observed a severe fuel leak on the starboard side generator and fuel strainer. CCSSC Ch. 2/ Reg 2.3

The arrangements for storage, distribution, and utilization of other flammable oils employed under pressure in power transmission systems, control, and activation systems and heating systems shall be such as to ensure the safety of the ship and persons onboard. PSCO observed the drip pan for the Fuel Transfer Pump to be full of fuel, and a leak emanating from the pump seal. CCSSC Ch. 3/Reg 3.20.4

The electrical power available shall be sufficient to supply all those services that are essential for safety in an emergency, due regard being paid to such services as may have to be operated simultaneously. The emergency source of electrical power shall be capable, having regard to starting currents and the transitory nature of certain loads, of supplying simultaneously at least the following services for a period specified hereinafter, if they depend upon an electrical source for their operation: PSCOs observed during the Emergency Generator test, the engine failed to start five times thus not properly supplying emergency power. CCSSC Ch. 2/ Reg 2.29.5

Fixed Fire Detection and Fire Alarm Systems of an approved type shall be installed in all machinery spaces, in all stairways, corridors and escape routes on new ships. PSCO’s observed multiple smoke alarms in machinery spaces and passageways with batteries disconnected and insufficient power supply. CCSSC Ch. 3/ Reg. 3.17.8

Electrical installation shall be such that: the safety of personnel and ship from electrical hazards will be assured. PSCO’s observed exposed wires in multiple locations in the engine room and hydraulic pump room which presented an electrical hazard to the crew. CCSSC CH. 2/ Reg. 2.26.1

For each ship...detail of fuel oil for combustion purposes delivered to and used on board shall be recorded by means of a bunker delivery note that shall contain at least the information provided in appendix V to this annex. Vessel could not provide PSCOs fuel bunkering receipts with sulfur content. 2017 MARPOL Annex I/18.5

The administration shall satisfy itself that the general structural strength of the hull is sufficient for the draught corresponding to the freeboard assigned. PSCO’s observed two insets exceeding 12 inches long and 2 ½ inches deep. PSCO’s also observed multiple bent frames, questioning the strength of the vessel’s hull. ICLL 66 Annex 1/Reg 1

The bulkheads and decks separating the machinery spaces of category A from control stations, corridors, accommodation spaces, stairway, service spaces and cargo spaces shall be so constructed as to be capable of preventing the spread of fire to the unexposed side. PSCO’s observed an exposed pipe penetrating the bulkhead between the engine control room and the port side of the engine room, rendering the space incapable of maintaining the fire boundary. CCSSC Ch. 3/Reg 3.17.1.2

Provision shall be made to prevent overpressure in any oil tank or in any part of the oil fuel system including the filling pipes. Relief valves and air or over-flow pipes shall discharge to a position which in the opinion of the Administration is safe. The open ends of air pipes shall be fitted with wire mesh. PSCO’s observed significant damage on the starboard side fuel vent wire mesh and piping. CCSSC Ch 3/Reg 3.20.2.5

In all ships other than barges with no under deck cargo spaces, means for sounding, to the satisfaction of the Administration, shall be provided... PSCO’s observed the sounding pipe of the forepeak ballast tank is broken. CCSSC Ch. 2/Reg 2.8.1

At least half of the number of lifebuoys referred to in 4.12.1 shall be fitted with self-igniting lights with technical specifications as outlined in the LSA Code. PSCO’s observed a lifebuoy located on the stern to be missing a light. After expanding the exam, PSCOs confirmed that the fire control plan requires light on the aforementioned lifebuoy. CCSSC Ch.4/Reg. 4.12.2

Every vessel shall have a Logbook on board. The master shall make or have made in the Logbook the following entries... PSCO’s were informed the vessel did not have a Logbook onboard and did not conduct pre-arrival and prior to departure checks. CCSSC Ch. 7/ Reg. 7.3.1

Every ship of 100 gross tonnage and above...shall carry a garbage management plan which the crew shall follow. PSCO’s observed incomplete and incorrect Garbage Record Book entries. In addition, Receptacles aren't labeled as per the Garbage Management Plan.

Whenever practicable, a gangway of not less than 20 inches (.51 m) in width, of adequate strength, maintained in safe repair and safely secured shall be used. PSCO’s observed the gangway had significant damage, cracks on welds, and missing bolts and screws, rendering the gangway unsafe for usage of personnel. 29 CFR 1918.22

Failure to comply with applicable safe manning requirements of the administration. The vessel is not manned as per the safe manning certificate. PSCO's observed the Chief Mate had a II/1 license which does not meet the M/V LA TEMPERANCE MINIMUM SAFE MANNING certificate requiring the vessel's Chief Mate to have a valid II/2 license. STCW I/4.2.2

Failure of seafarers to hold a certificate, to have an appropriate certificate, to have a valid dispensation or to provide documentary proof that an application for an endorsement has been submitted to the administration. PSCO's observed the master's medical certificate had been tampered with by whiting out the issue and expiration dates. The original expiration date is March 24, 2023. The issue date is March 24, 2021. PSCO's were unable to verify the validity of the medical certificate. Without a valid medical certificate, the Master is not fit for duty for operations of the vessel and the license is invalid. STCW I/4.2.1

Cargo, cargo units, and cargo transport units carried on or under deck shall be so loaded, stowed, and secured as to prevent as far as practicable, throughout the voyage, damage, or hazard to the ship and the persons onboard, and loss of cargo. PSCO's observed cargo inside the vehicle loaded onboard the vessel. Vessel visibility from the birdge is impaired and both forward passageways in the cargo spaces arfe impared by cargo. Additionally, cargo is not adequately secured to the ship. 74SOLAS(20 Cons.)VI/5.1 17C

Rescue boats should be stowed in a state of continuous readiness for launching in not more than 5 minutes. PSCOs observed rescue boat engine failed to remain running after initial start.

All machinery and electrical installations, mechanical and electrical equipment and appliance...should be of design and construction adequate for the service for which they are intended and should be so installed and protected as to reduce a minimum any danger to persons on board; due regard being paid to moving parts, hot surfaces and other hazards. PSCOs observed fuel pooled around multiple fuel lines and wet fuel injectors.

The company should establish procedures to ensure that the ship is maintained in conformity with the provisions of the relevant rules and with any additional requirements, which may be established by the company. Due to the objective evidence in the identified deficiencies, the vessel is not in substantial compliance with relevant conventions, calling into question the adequacy and implementation of the vessel’s SMS under the ISM Code. PSCO’s collected objective evidence that the Minimum Safe Manning Certificate is expired, the Load Line Certificate is invalid and the Caribbean Cargo Ship Safety Certificate Supplement is invalid and not updated with equipment currently onboard. Furthermore, Cargo Securing Manual requires securing devices to be inspected via routine visual examinations, periodic examinations and daily examinations while vessel is underway for damage and service as needed before and after each use. PSCOs collected objective evidence that the Cargo Securing manual is not being followed. In addition, the Master of the vessel is not following proper procedures for logging requirements as required by the SMS. An external audit conducted by the Flag is required to determine whether the ship is operating in accordance with the ISM Code. Provide a copy of the completed audit report to USCG prior to departure from port. 74 SOLAS (20 Cons.) IX/3.1 ISM Code A/10.1

The International Load Line Certificate shall not be delivered to the ship until the officer or surveyor acting under the provisions of article 13 of the Convention has certified that the marks are correctly and permanently indicated on the ship's sides. Load Line Certificate does not have correct markings corresponding with the ship. ICLL 66 Reg 9

Cargo Securing Manual requires securing devices to be inspected via routine visual examinations, periodic examinations and daily examinations while vessel is underway for damage and service as needed before and after each use. PSCO's observed no records or maintenance of any servicing that had been conducted since February 10, 2020.

For every ship to which Chapter 1 applies, the Administration shall: issue an appropriate minimum safe manning document as evidence of the minimum safe manning considered necessary to comply with the provisions of paragraph 1. PSCOs observed that the Minimum Safe Manning Certificate is expired. CCSC 1.7.2

Any alterations, modifications or renewals of ship board equipment as a result of the survey referred to in 1.10.1.2 shall be recorded in the ship's Record of Equipment and Ship Information, which shall be kept on board the ship. A copy of the results of the latest surveys referred to in 1.10.1.2 to 1.10.1.6 shall be kept on board ship. The annual service dates for each life raft are not accurately recorded on the ship's Record of Equipment and Ship Information. Additionally, the SART on the starboard side is expired.

Electrical installations shall be such that: the safety of personnel and ship from electrical hazards will be assured. PSCO’s discovered exposed wiring on starboard side of the ship outside the bridge creating an electrical hazard for the crew. CCSC 2.26.1

Fixed fire detection and Fire Alarm Systems of an approved type shall be installed in periodically Unattended machinery spaces on existing ships. One Smoke Alarm was inoperable in the Steering Gear Room, an unattended Machinery Space, leaving the space without any fire detection CCSC 3.17.7

Any oil or other substances flammable or harmful to the marine environment should be carried in other tanks or spaces which are not specially approved by the administration for such purposes. PSCO's discovered multiple buckets filled with oil stored in the engine room creating a hazardous environment for the ship’s crew and posing a critical fire hazard for the vessel. CCSC 3.20.8

Lifebuoys shall be marked in block capitals of the Roman Alphabet with the name and port of registry of the ship on which it is carried and fitted with retro-reflective material. PSCO’s observed two lifebuoys not marked with the port of registry or the name of the ship. CCSC 4.12.5.3

The condition of the ship and its equipment is maintained to conform with the provisions of this code to ensure that the ship in all respects will remain fit to proceed to sea without danger to the ship, person on board, or the environment. PSCO's discovered while the vessel is underway, the crew removes the MDE's cylinders to manually lubricate causing an unsafe environment in the engine room. CCSS 1.11.1

Movings parts of machinery which are so exposed to cause accidents shall be properly guarded. Engines cylinder cover were removed during operation creating an unsafe operation. CCSS 2.12.6

Every ship shall be provided with at least two anchors of sufficient weight... Vessel lost both of it anchors. CCSSC CH 2.9.1

Fire Control systems should be kept to date. EEBD in workshop is inoperable. CCSSC 3.13.2

The strength and construction of the vessel's structure should be sufficient to withstand all foreseeable conditions. Ceiling in dry stores room is damaged. CCSSC 2.4.1

The owner or master of every ship should ensure that the condition of the ship and its equipment is maintained to remain fit to proceed to sea. The main valve for fuel tank #2 storage is inoperable. CCSSC 1.11.1

The bulkheads and decks separating the machinery spaces of category A from control stations, corridors, accommodation spaces, stairways, service spaces and cargo spaces shall be so constructed as to be capable of preventing the spread of fire to the unexposed side. Unsealed penetration of the engine room bulkhead at the stairs to the bridge allow for the passage of smoke and flame. C.C.S.S.C CH.3 Reg. 3.17.1.2 17bc

The arrangement of each ventilation system shall be such that fire in one space shall not readily spread to the other spaces. Crew created an unaut¬horized 8 in. x 20 in. cut-out in the bulkhead inboard of the Port side ship service diesel generator. This cutout creates an opening to the ventilation duct leading from the main engine space one deck below. When this cut-out is uncovered, there is no way to prevent the passage of smoke or fire between the two spaces. C.C.S.S.C CH.3 Reg. 3.17.6.1 17bc

All ships shall be provided with a permanently exhibited fire control plan or equivalent to the satisfaction of the Administration. Fire control plan identifies the boundaries to the galley as “A” class boundaries. Neither of the 3 doors allowing for movement through the boundary openings have markings to indicate that they meet the standards for an “A” class boundary. Additionally, the stbd door on the forward bulkhead of the galley has no latching mechanism, preventing the crew from being able to keep the door closed. C.C.S.S.C CH.3 Reg. 3.13.1 17bc

The position of the rudder, if power-operated, shall be indicated at the navigation bridge. PSCO on the bridge observed a reading of 26 degrees starboard while PSCO at the steering gear observed a reading of approximately 35 degrees. Additionally, bridge rudder angle indicator shows a range of 40 degrees port to 40 degrees starboard while indicator on the steering gear only ranges 30 degrees in either direction. C.C.S.S.C CH.2 Reg. 2.33.8 17bc

The main steering gear shall be of adequate strength and sufficient to steer the ship at maximum ahead service speed. During steering test vessel master put the rudder hard to port at 40 degrees. Master then attempted to put the rudder hard to starboard and could only achieve 26 degrees on the rudder. Both readings were taken at the rudder angle indicator located on the bridge. C.C.S.S.C CH.2 Reg. 2.33.3 17bc

Owners and operators of non-party vessels must keep these records as specified in…regulations 13, 14, and 18 of 2008 MARPOL Annex VI…Vessel could not produce bunker delivery notes to accompany invoice for 1,220 gallons of fuel bunkered on 7/2/2021. 40 CFR 1043.70(a) 17bc

The master shall ensure that the frequency of drills performed shall not be less than the following frequencies if weather and circumstance permit: Enclosed Space Entry- Quarterly. Last recorded enclosed space entry was noted in June 21. Vessel has not completed subsequent enclosed space entry drill since coming out of dry dock in September 21. CSCCS Ch. 7 Reg. 7.4.3.2 17bc

The log entry required by 7.3.1.5 in respect of …entry into enclosed spaces-and fire drills shall include the following information: Date of the drill and training; and general description of the drill scenario and training topics. The identified enclosed space entry drill that took place in June 2021, was only a completed checklist for entering an enclosed space, and did not clearly specify if it was drill, what space was involved and any covered training for the scenario. CSCCS Ch. 7 Reg. 7.3.2.1-.2 17bc

The master shall ensure that the frequency of drills performed shall not be less than the following frequencies if weather and circumstance permit. Enclosed Space Rescue- bi-annually. No documentation was available to show completion of enclosed space rescue drill since the vessel returned to service. CSCCS Ch. 7 Reg. 7.4.3.3 17bc

All ships of 300 GT and over and all ships under 300 GT and assigned to the Unrestricted Area to which this code applies shall comply with the provisions of Chapter IV of the SOLAS convention as amended. Every ship shall be provided with: a receiver capable of receiving international NAVTEX service broadcasts. Vessel had a NAVTEX apparatus on board, but when it was powered on it showed no data for appropriate broadcast information. CSCCS Ch. 5 Reg. 5.1.1 SOLAS 74’ (2020 Cons) IV/7.1.4 17bc

In addition to complying with the provisions of this section, every ship shall comply with any other requirements which, in the opinion of the Administration, are deemed necessary to prevent accidents at sea and to maintain appropriate living and working conditions. PSCO’s observed a moldy, inoperable refrigerator which the crew claimed was still used for food storage. C.C.S.S.C CH.2 Reg. 2.12.8 17bc

In all such ships, fire control plans shall be kept up to date. Vessel has three fire control plans on board. All of them reflect different information about the fire protection on board the vessel and have different markings for approving authority. In addition, the symbols depicted in the key reflected one color, but were a different color on the drawing of the ship. One fire control plan did not show the location of the galley, and none of the plans showed the location of the emergency batteries as indicated in the legend. CSCCS Ch. 3 Reg. 3.13.2 17bc

A classification society (including an employee or agent of that society) may not review, examine, survey, or certify the construction, repair, or alternation of a vessel in the United States unless the society has applied for approval under this subsection and the Secretary has reviewed and approved that society with respect to the conduct of that society under paragraph (2). Surveyor who approved dry dock and steelwork is not approved per this section. 46 U.S.C 3316(c)(1) 17bc

Ships to which this chapter applies shall carry not less than the number of lifebuoys according to the following table: length of ship in meters 50 and over, minimum number of lifebuoys 8. PSCO found 01 lifebuoy that was cracked with interior structure of the buoy exposed and no longer serviceable. CSCCS Ch. 4 Reg. 4.12.1 17bc

Every ship over 100 GT and ship certified to carry 15 persons or more shall carry a Garbage Management Plan. Vessel was issued a Garbage Management Plan approval letter dated 15OCT2018. However, the Garbage Management Plan is dated August 2020. CSCCS Ch. 7 Reg. 7.5.4 16bc

The company and the ship shall comply with the requirements of the International Safety Management Code. The company should establish program for drills and exercises to prepare for emergency actions. During the review of the SMS procedures for enclosed space entry drills, the SMS referenced using form number S-14. When PSCO’s referenced form number S-14 it was labeled “Personnel Accident-Injury Report.” SOLAS 74 (2020 CONS) IX/ 3.1 ISM Code 8.2 50bc

The company should establish procedures, plans and instructions, including checklists as appropriate, for key shipboard operations concerning the safety of the personnel, ship, and safety to the environment. CH. Engineer did not obtain bunker notes showing Sulphur content of fuel received on 7/2/2021 as required on page 27 of vessel’s SMS. ISM Code Part A. Reg. 7 74 SOLAS (2020 CONS) CH. IX Reg. 3.1 50bc

The company should establish procedures, plans and instructions, including checklists as appropriate, for key shipboard operations concerning the safety of the personnel, ship, and safety to the environment. CH. Engineer did not make an entry in the Oil Record Book for 1,220 gallons of fuel received on 7/2/2021 as required on page 27 of the vessel’s SMS. ISM Code Part A. Reg. 7 74 SOLAS (2020 CONS) CH. IX Reg. 3.1 50bc

Every life raft shall be stowed with its painter permanently attached to the ship and with a float-free arrangement (weak link) so that the life raft floats free. PSCO found both port and starboard side life rafts were not set-up in a float-free arrangement. CSCC Ch. 4 Reg. 4.9.5 17c

EVERY SHIP SHALL BE PROVIDED WITH AT LEAST ONE INDEPENDENT POWER-OPERATED FIRE PUMP CAPABLE OF DELIVERING A JET OF WATER AS REQUIRED BY REGULATION 3.3.2. Main and emergency switchboards shall be so arranged as to give easy access as may be needed to apparatus and equipment without danger to personnel. The switchboards shall be suitably guarded as deemed necessary by the administration. PSCO observed exposed wiring on main service fire pump control panel and electrical panel located at lube oil tank. CCSSC 27.2

Exposed metal parts of electrical machines or equipment which are not intended to be live but which are liable under fault conditions to become live shall be earthed. PSCO observed unprotected and uncovered alternator shaft attached to pump drive belt. CCSSC 2.27.1.1

All machinery and electrical installations, shall be so installed and protected as to reduce to a minimum any danger to persona onboard, due regard being paid to moving parts. PSCO observed generator 1 missing multiple guards over moving parts. CCSSC 2.3

All machinery and electrical installations shall be so installed and protected as to reduce to a minimum any danger to persons onboard, due regard being paid to hot surfaces. PSCO observed no lagging installed on hot exhaust surfaces. CCSSC 2.3

All machinery and electrical installations shall be so installed and protected as to reduce to a minimum any danger to persons onboard, due regard being paid to other hazards. PSCO observed excessive amounts of oil in the bilge. CCSSC 2.3

All electrical apparatus shall be so constructed and so installed as to not cause injury when handled or touched in a normal manner. PSCO observed numerous locations throughout the vessel that contained dead end wiring. CCSSC 2.27.1.3

No person may cause a vessel to enter the navigable waters of the united states unless no mare than 12 hours before entering the vessel completes equipment checks in accordance with 33 CFR 164.25(a). Vessel did not conduct pre-arrival equipment checks. 33 CFR 164.25(a)

Each separate circuit should be protected against short circuit and against overload. Vessel experienced a short circuit which caused all lights to go out. An overload to every installation on the 220V circuit occurred with no known cause. CCSSC 2.31.6.1 17bc

Whenever a defect is discovered on a ship a request should be made to the Administration responsible and a relevant survey should be carried out as soon as practicable. Void spaces in the structural arrangement of the hull have been discovered to contain an unknown quantity and type of liquid. CCSSC 1.11.3 17bc
