74 SOLAS 20 II-2/9.4.1.1.2: The construction of doors and door frames in "A" class divisions, with the means of securing them when closed, shall provide resistance to fire as well as to the passage of smoke and flame equivalent to that of the bulkheads in whcih the doors are situated. PSCO observed roller shutter fire doors 19-2-08 and 15-7-15 not fully closed upon remote activation, resulting in 1 inch gap from deck.
Action required: 16 - Rectify deficiencies w/in 14 days

74 SOLAS 20 II-2/9.4.1.1.5.2: Fire doors in main vertical zone bulkheads, galley boundaries and stairway enclosures, the approximate uniform rate of closure for sliding doors shall be no more than 0.2 m/s and no less than 0.1 m/s. PSCOs observed unsatisfactory closure of main vertical zone fire door 06-4-03 dual rolling shutter arrangement. Forward rolling shutter stopped 3 times during closure and it took 95 seconds to close completely.
Action required: 16 - Rectify deficiencies w/in 14 days

74 SOLAS 20 II-9/9.4.1.1.5.12: Fire doors in main vertical zone bulkheads, galley boundaries and stairway enclosures, double-leaf doors equipped with a latch necessary for their fire integrity shall have a latch that is automatically activated by the operation of the doors when released by the system. PSCO observed stairway enclosure double-leaf fire door not latching properly in sequence.
