Boiler space ventilation closure flap on entrance door, does not close fully, allowing for fire extinguishing agent to escape to atmosphere.

All engines must have at least two means for stopping the engine(s) under any operating conditions. The fuel oil shutoff required at the engine by § 182.455(b)(4) will satisfy one means of stopping the engine. Fuel shutoff ball valves for stbd engines and generator do not close completely.

Number of child size life jackets equal to at least 10% of the number of persons permitted on board must be provided. Vessel is missing 01 child life jackets. Vessel maximum passengers requiring child life jackets (90 lbs or less)is reduced to 12.

The construction and arrangement of a vessel must allow the safe operation of the vessel in accordance with the terms of its Certificate of Inspection. Crack identified on shaft support leg mount for engine #4. Make permanent repairs to original.

Piping used in a vital system must: (1) Be composed of ferrous materials except when: (i) Nonmetallic piping materials are permitted by § 182.720... (Rigid nonmetallic materials (plastic) may be used only non-vital systems. Exhaust cooling water from from all engines has section (flow sensor) of piping that is constructed of rigid nonmetallic plastic. Provide repair plan and make permanent repairs.

All vessels must have navigation lights that are in compliance with the applicable sections of the International and Inland Navigation Rules, except that a vessel of more than 19.8 meters (65 feet) in length must also have navigation lights that meet UL 1104, “Standards for Marine Navigation Lights,” or other standard specified by the Commandant. STBD Nav light inoperable.

A pre-engineered fixed gas fire extinguishing system must:(3) Automatically shut down all power ventilation systems and all engines that draw intake air from within the protected space. Boiler space has a small vent that cannot be secured. Request a proposal on how to secure ventilation to satisfaction of USCG MI.

All machinery and equipment must be installed and maintained in such a manner as to afford adequate protection from fire or machinery failure. STBD oil pressure sensor on jet drive is being supported by zip ties. Repair to satisfaction of USCG.

A vessel to which this subchapter applies may not be operated without having on board a valid U.S. Coast Guard Certificate of Inspection. Vessel was found with a expired COI. 46 CFR 176.100

07110 - Spanner wrenches must be provided where a 40 millimeter (1.5 inch) diameter firehose is required. Fire hose station does not have a spanner wrench. 46CFR181.310

04116 - On a vessel of more than 19.8 meters (65 feet) in length, the public address system must be a fixed installation and be audible during normal operating conditions. Aft PA speaker is not operational. 46CFR184.602

03103 - Piping, including valves, pipe fittings and flanges, conveying vapor, gas, or liquid, the temperature of which exceeds 65.5 °C (150 °F), must be suitably insulated where necessary to prevent injuries. Port aft engine lagging is compromised. 46CFR177.970

01326 - When, in accordance with § 178.210 of this chapter, a vessel must be provided with a stability letter, the stability letter must be posted under glass or other suitable transparent material, such that all pages are visible, at the operating station of the vessel. Onboard stability letter is missing page 1. 46CFR178.210

01199 - The Certificate of Inspection and any SOLAS Certificates must be posted under glass or other suitable transparent material, such that all pages are visible, in a conspicuous place on the vessel where observation by passengers…Posted COI is not the most up to date. 46CFR176.302

All machinery and equipment must be installed and maintained in such a manner as to afford adequate protection from causing fire, explosion, machinery failure, and personnel injury. Properly repair the jacket water leak on the starboard outboard engine to the satisfaction of the attending CG Marine Inspector. 46 CFR 182.100

At each inspection for certification and at every other vessel inspection the bilges and other spaces may be examined to see that there is no excessive accumulation of oil, trash, debris, or other matter that might create a fire hazard, clog bilge pumping systems, or block emergency escapes. Found flammable liquids and aerosols in the vessels tool storage room located amidships on the main deck. Remove all combustibles from the vessel or provide an approved flammable storage cabinet onboard within 48 hours. 46 CFR 176.830

Each survival craft must be secured to the vessel by a painter with a float free link permanently attached to the vessel and stowed so that when the vessel sinks the survival craft floats free and if inflatable, inflates automatically. The starboard IBA painter line was not connected to hydrostatic releases week link. Install Starboard IBA painter line to the week link on the hydrostatic release unit. 46 CFR 180.130.

EPIRB must be installed to automatically float free and activate if the vessel sinks. Vessel hydrostatic release was expired. Replace Hydrostatic release. 46 CFR 180.64.

Each ring life buoy must be marked in clearly legible block capital letters with the vessels name. Two of the three ring life buoys name had faded away. Properly stencil vessels name onto the ring life buoys. 46 CFR 185.604(b).

The person in command of a documented vessel must have on board that vessel the original Certificate of Documentation (COD) currently in effect for that vessel. Vessel had onboard a photo copy of the current COD. Keep the original COD onboard the vessel. 46 CFR 67.313.

A vessel must carry adequate and up to date, US COAST PILOT, extracts from the publication may be provided instead of the the complete publication. 46 CFR 184.420.

Provide update charts for area of operations.
