Repairs or alterations to the hull, machinery, or equipment that affect the safety of the vessel must not be made without the approval of the cognizant OCMI, except during an emergency. When repairs are made during an emergency, the owner, managing operator, or master shall notify the OCMI as soon as practicable after such repairs or alternations are made. Repairs or alterations that affect the safety of the vessel include, but are not limited to: replacement, repair, or refastening of deck or hull planking, plating, and structural members; repair of plate or frame cracks; damage repair or replacement, other than replacement in kind, of electrical wiring, fuel lines, tanks, boilers and other pressure vessels, and steering, propulsion and power supply systems; alterations affecting stability; and repair or alteration of lifesaving, fire detecting, or fire extinguishing equipment. A report of allision was provided. Allision resulted in outboard motor dismounting from transom and into the water. Conduct full damage survey to include the following: possible hull damage, (divers, unless hauled out) propulsion, steering, electrical, fuel line, throttle cable, and all other associated equipment. Provide a written damage survey to the marine inspector with all findings; Including paperwork from Offshore Marine. Provide written repair plan to the marine inspector. Schedule a deficiency check with the marine inspector to conduct sea trials prior to the carriage of passengers.

Prior to operations with passengers root cause analysis shall be conducted, repairs made to the engine and vessel must undergo sea trials with a Coast Guard marine inspector present. 46 CFR 176.500 (iv)

When new vessel management personnel and operators are hired the owner shall insure that the new hires are familiar with Coast Guard marine causality definitions and legal reporting requirements found in 46 CFR 4.03-1 and 46 CFR 4.05-1 for Coast Guard inspected vessels. 46 CFR 4.03-1 46 CFR 4.05-1

Portable fire extinguisher must be located so that they are clearly visible and readily accessible from the space being protected. The installation and location must be to the satisfaction of the Officer in Charge of Marine inspection. Attending MI noted that all fire extinguisher were loose on the deck where passengers could trip over them. Mount all extinguisher in acceptable brackets. 46 CFR 181.520

Repair or replace the water light attached to the required ring life buoy. Attending MI noted that it was inoperable at the time of the inspection. 46 CFR 180.70

Fuel tank does not have fuel shutoff valve installed.

Label fuel shutoff valve with 1" lettering.

Install gauges to indicate RPM, jacket water discharge temperature, and lubricating oil pressure at the operating station.

Provide current DAPI covered employee list, designated employee representative (DER), and current crewmember training list.

Provide FCC Station license and emergency broadcast placard at the operating station.

Vessel did not undergo an annual inspection within the three months before or after each anniversary date. Vessel is out of the water and not operating and will have to undergo an annual inspection prior to operating with passengers.

Provide one(1) 40 B:C Fire extinguisher for the outboard engine and one (1) 2A extinguisher for the accommodation space with the CG approved brackets.

Provide one (1) serviceable life ring with a lifeline, floating waterlight with a corrosion resistant metal clip per 46 CFR 180.70

PFD stowage is not properly marked and adult and child PFDs are stowed together on the overhead. Properly mark PFD stowage and separate child and adult PFDs

Label lifejacket storage boxes with quantity of adult and child PFDs.

Label lifejactkets with vessel name.

Provide first aid kit with required contents found in 46 CFR 160.041.

Provide battery for lifebuoy waterlight.

Provide 3'-6' lanyard with corrossion resistant clip for lifebuoy waterlight.

Provide 3 firebuckets of 2 1/2 gallon capacity.

Post revised emergency instructions that reflect vessel arrangement and procedures.

Provide tides, current tables, coast pilot and light list for operating area.

Provide and post garbage placard appropriate for operating area IAW 33 CFR 151.59 and 33 CFR 151.71.

Provide Employee Assistance Program phone numbers and training records.

Provide drug and alcohol program records for all employees.

Vessel limited to 1000 ft from shore until FCC Station License, FCC Safety Radiotelephony Certificate are provided for vessel, General marine Operators Permit provided for each captain, emergency broadcast placard, and MMSI number and GPS input provided for VHF DSC marine radio.

Vessel's new owner will be required to complete a "new-to-zone" inspection once the vessel is delivered to its new OCMI zone. The attached CG-835, and the associated worklist, will need to be completed to the satisfaction of the attending MI.
