Shutoff valves, installed so as to close against the fuel flow, must be fitted in the fuel supply lines, one at the tank connection and one at the engine end of the fuel line to stop fuel flow when servicing accessories. The shutoff valve at the tank must be manually operable from outside the compartment in which the valve is located, preferably from an accessible position on the weather deck. The starboard side fuel-shut-off valve did not closed completely. Prove the full closing of the fuel shut-off valve. 46 CFR 182.455 (b)(4) Code 17C.

The construction and arrangement of a vessel must allow the safe operation of the vessel in accordance with the terms of its Certificate of Inspection giving consideration to provisions for a seaworthy hull. Area of FRP fractures/delamination were found in the collision bulkhead, under deck, and starboard side of the side shell and collision bulkhead as indicated during the exam. Conduct proper FRP repairs to the satisfaction of an attending USCG Marine Inspector. 46 CFR 177.100 Code 50 (30 Days).

On a vessel of at least 26 feet in length, a visual and audible alarm must be provided at the operating station to indicate a high water level in A space with a through-hull fitting below the deepest load waterline, such as a lazarette. High water bilge both visual and audio alarms were found inoperable in the steering compartment (lazarette). 46 CFR 182.530(a)(1). Code 17C.

A visual indicator must be provided at the operating station to indicate when any automatic bilge pump is operating. The steering compartment bilge pump manual operation actuator button and LED indicator at the operating station was found inoperable. 46 CFR 182.530(a)(c), code 17C.

All engines must be water cooled and The engine head, block, and exhaust manifold must be water-jacketed and cooled by water from a pump that operates whenever the engine is operating. Starboard side main diesel engine heat exchanger was found to have a coolant water leak. 46 CFR 182.420(a)(1), Code 17C.

A hatch exposed to the weather must be watertight. Forward, mid and aft hatches were found leaking water inside the compartment upon testing with water poured on top with the fire buckets. 46 CFR 179.360 Code 17C.

A vessel on which a pre-engineered fixed gas fire extinguishing system is installed must have the following equipment at the operating station:(1) A light to indicate discharge;(2) An audible alarm that sounds upon discharge; and(3) A means to reset devices used to automatically shut down ventilation systems and engines as required by paragraph (a)(3) of this section. As stated by the master, the pre-engineered fixed gas fire suppression system operation controls at the operating station was found inoperable prior the COI exam being conducted in the morning, and replaced prior ending the exam. Control were tested and found fully operational. 46 CFR 181.420 (b) Code 17C.

For fixed-gas fire extinguishing systems, the owner or managing operator must provide satisfactory evidence of the required servicing to the marine inspector. If any of the equipment or records have not been properly maintained, a qualified servicing facility may be required to perform the required inspections, maintenance procedures, and hydrostatic pressure tests. Provide a fire servicing report attesting the type of inspection or examination was conducted to the pre-engineered fire suppression system. 46 CFR 176.810 (b)(1), code 16C.

On a vessel on which the number of passengers permitted on upper decks is limited by stability criteria, as indicated by the vessel's stability letter, the maximum number of passengers allowed on an upper deck must be indicated by a durable marking of at least 1 inch numbers and letters at the entranceway to that deck. Provide by the upper deck entranceway with maximum allowed number of passengers permitted in the upper deck. 46 CFR 185.602 (g), code 16 C.

A drip pan fitted with flame screen must be installed under gasoline strainers. Fuel filter and strainer bowls must be highly resistant to shattering due to mechanical impact and resistant to failure due to thermal shock. Fuel filters does not have fire resistant bowls. 46 CFR 182.455 (6), coe 16C

At each inspection for certification and at every other vessel inspection all observed hazardous situations must be corrected and all required guards and protective devices must be in satisfactory condition. Starboard side aft deck stanchion was found broken/loose at its base. 46 CFR 176.830, code 16C.

Electrical equipment and installations must be suitable for the roll, pitch, and vibration of the vessel underway. Dead-end wires were found in the middle compartment, as identified during the exam. 46 CFR 183.220, code 16C.

All machinery and equipment must be installed and maintained in such a manner as to afford adequate protection from causing machinery failure, and personnel injury. The temperature gauge at the operating station for the starboard side main diesel engine was found not working properly. 46 CFR 182.100, code 16C.

When operating a vessel on a body of water where the discharge of untreated sewage is prohibited by the Environmental Protection Agency under 40 CFR 140.3 he operator must secure each Type III device in a manner which prevents discharge of sewage. Acceptable methods of securing the device include—(1) Closing each valve leading to an overboard discharge and removing the handle;(2) Padlocking each valve leading to an overboard discharge in the closed position; or(3) Using a non-releasable wire-tie to hold each valve leading to an overboard discharge in the closed position. Type III sewage holding tank was found leaking into the bilge. 33CFR 159. Code 16C.

The construction and arrangement of a vessel must allow the safe operation of the vessel in accordance with the terms of its Certificate of Inspection giving consideration to provisions for a seaworthy hull, protection against fire, means of escape in case of a sudden unexpected casualty, guards and rails in hazardous places, ventilation of enclosed spaces, and necessary facilities for passengers and crew. Engine room forward and aft bulkheads were found to have penetrations with electrical cables leading to aft steering and mid compartments without proper seal. 46 CFR 177.100 code 16C.

Each vessel that operates beyond three miles from the coastline must have on board a FCC Type Accepted Category 1, 406 MHz EPIRB. EPIRB battery on board expired in February 2023. 46 CFR 180.64

Each floating waterlight on a vessel carrying only one ring buoy, must be attached to the lanyard with a corrosion-resistant clip. The clip must have a strength of at least 50 pounds and allow the waterlight to be quickly disconnected from the ring life buoy. Ring buoy on board was missing corrosion-resistant clip. 46 CFR 180.70 d(3)

050109: Title 47 Code of Federal Regulations (CFR) Part 80, requires that all passenger vessels, regardless of size, must be in compliance with the rules applicable and operating in Sea Area A1, must have upgraded to VHF-DSC Class A radio, either properly connected to a GPS receiver or have a VHF-DSC Marine Radio with an integral GPS. Vessel VHF-DSC radio installed on board was found not displaying GPS coordinates. 46CFR Part 80. 16C.

99101: The cognizant OCMI may require that a vessel and its equipment undergo any additional test or inspection deemed reasonable and necessary to determine that the vessel and its equipment are suitable for the service in which they are to be employed. The portside handrail, following the portside stern diver, its support base was found lose with minor damage to the FRP under deck. 46 CFR 176.840

02108: On a nonmetallic vessel, the enclosures and frames of electrical equipment must be bonded together to a common ground by a normally non-current carrying conductor. Vessel's ground wires was found disconnected, damaged due to corrosion of the terminal and cable insulation material was found deteriorated in steering, engine and mid compartments. 46 CFR 183.372

02108: All cable and wires must be protected from the weather. Clean electrical cable connectors and replace those that were found corroded at the terminal strips in mid compartment and engine rooms. 46 CFR 183.340.

13103: Gauges to indicate engine revolutions per minute (RPM), jacket water discharge temperature, and lubricating oil pressure must be provided for all propulsion engines installed in the vessel. The gauges must be readily visible at the operating station. Gauges at the operating station were found with protective cover damaged, and other hanging underneath the operating station. 46 CFR 182.410(b).

02101: A hatch exposed to the weather must be watertight. Clean all three watertight hatches rubber gasket seals and the frame on which it makes contact. 46CFR179.360.

07109: A space containing propulsion machinery bust be equipped with a fixed gas fire extinguishing system. Engine room bulkhead penetrations must be sealed to prevent the passage of air inside the protected space. 46CFR181.400.

Each item of lifesaving equipment determined by the marine inspector to not be in serviceable condition must be repaired or replaced. The vessel life float was discolored and over 20 % of the vinyl layer was deteriorated. The retro reflective SOLAS tape was no longer reflective and requires new tape on all sides of the life float. The metal ring within the center webbing was broken.

The float-free link must be secured to the painter at one end and to the vessel on the other end. The float-free link was not attached to the vessel.

Each ring life buoy must be marked with Type II retro-reflective material. The ring life buoy was missing the required retro-reflective tape.

Each disposable hydrostatic release unit must be marked with an expiration date of two years after the date of which the unit is installed. A newly installed hydrostatic release unit for the EPIRB was not properly marked with the expiration date.

A vessel not required to have a power pump must three 2.5gal fire buckets with attached lanyard. The words "FIRE BUCKET" must be stenciled in a contrasting color on each bucket. One fire bucket was not properly stenciled, and all three fire buckets were missing a lanyard.

Each vessel must be equipped with at least two operable portable battery lights. The vessel only had one operable flashlight.

Each vessel that operates on the high seas, or that operates beyond three miles from the coastline of the Great Lakes, must have on board a FCC Type Accepted Category 1, 406 MHz EPIRB, installed to automatically float free and activate. The vessel's hydrostatic release unit was expired. Vessel is limited to voyages not more than 3 NM from shore.

A vessel on which a pre-engineered fixed gas fire extinguishing system is installed must have the following equipment at the operating station: (1) A light to indicate discharge. The visual light located at the operating station was inoperable.

On a vessel of at least 7.9 meters (26 feet) in length, a visual and audible alarm must be provided at the operating station to indicate a high water level in each of the following normally unmanned spaces. The visual alarm located at the operating station for the steering compartment is inoperable.

On a vessel on which the number of passengers permitted on upper decks is limited by stability criteria, as indicated by the vessel's stability letter, the maximum number of passengers allowed on an upper deck must be indicated by a durable marking of at least 25 millimeters (1 inch) numbers and letters at the entranceway to that deck. The maximum number of passengers permitted on the second deck was not marked.

At each vessel inspection all observed unsafe practices and other hazardous situations must be corrected. Two (02) FRP fractures were found that require repair: 1)Fracture at the steering compartment overhead in-way-of (IWO) the port stringer; and 2)fracture at the top left corner of the center transverse open bulkhead.

At each vessel inspection all observed unsafe practices must be corrected. There was an opening located at the starboard quarter at the bulwarks that allowed water to enter and accumulate within the swim platform.

Propulsion machinery must be suitable in type and designed for propulsion requirements of the hull in which it is installed and capable of operating at constant marine loads under such requirements without exceeding its designed limitations. Vessel captain indicated that unusual vibrations occurred when the vessels was being operated at higher RPMs. Prove proper operation of both port and starboard main diesel engines upon satisfaction of an attending USCG Marine Inspector, prior departure with passengers for hire.

A submersible electric bilge pump may be used as a power bilge pump only on a vessel of not more than 19.8 meters (65 feet) in length carrying not more than 49 passengers, provided that the pump is listed by an independent laboratory as meeting the requirements in UL 1113. UL reference was not present on submersible bilge pump located in steering compartment. Owner/operator shall provide documentation of existing pump that meets requirements of UL1113 or replace with one that does.

A vessel of more than(26 feet)in length, but not more than(65 feet), must carry a minimum of one life buoy of not less than (24 inches) in diameter. At least one ring life buoy must be fitted with a lifeline. Each lifeline on a ring life buoy must be buoyant, be of at least (60 feet) in length, be non-kinking, have a diameter of at least (5/16 inch), have a breaking strength of at least(1,124 pounds), and be of a dark color if synthetic, or of a type certified to be resistant to deterioration from ultraviolet light. Repair or replace worn and deteriorated 24" Life Bouy and lifeline.

The master and crew of a vessel will be familiar with the content of and have mounted at the operating station, emergency instructions containing the actions to be taken in the event of fire, heavy weather, or man overboard conditions. Emergency instructions were not mounted at operating station during inspection.

A vessel certificated to operate on a limited coastwise route in warm waters must be provided with life floats that will accommodate at least 50% of the total persons permitted on board. The vessel was found to be fitted with buoyant apparatuses approved under 160.010 which are not permitted. Vessel is restricted to operations not further than 3 miles from shore until the vessel is fitted with life floats approved under 160.027.

Vessels must comply with the applicable FCC regulations contained in 47 CFR Part 80. The vessel was found to be missing the FCC Station License.

The name of the vessel must be marked in clearly legible letters on each life float paddle. The required markings on the paddles were missing.

Machinery and equipment on the vessel must be suitable for the vessel and its operation and for the purpose intended. The port side fuel tank flexible hose fitting was found to be loose and showed signs of a small diesel leak.

Cable and wiring must be installed with metal supports spaced not more than 610 mm (24 inches) apart in such a manner as to avoid chafing. Observed bundles of wiring in sewage tank space secured with zip ties. Install metal supports every 610 mm.
