Repairs or alterations to the hull, machinery, or equipment that affect the safety of the vessel must not be made without the approval of the cognizant OCMI, except during an emergency. The owner or managing operator shall submit drawings, sketches, or written specifications describing the details of any proposed alterations to the cognizant [name withheld] alterations must be approved by the OCMI before work is started. Vessel replacing main engines. Request specifications of new engines. Sea trial required after install and prior to taking on passengers. 46 CFR 176.700

The material, design, construction, workmanship, and arrangement of main propulsion machinery and of each auxiliary, directly connected to the engine and supplied as such, must be at least equivalent to the standards established by the ABS Steel Vessel Rules (incorporated by reference, see 46 CFR 58.03-1), except as otherwise provided by this subchapter. The outer port propeller is experiencing vibrations.

Each child size life jacket must be stowed in a location that is appropriately marked and separated from adult life jackets so the child size life jackets are not mistaken for adult life jackets. Child size life jackets do not indicate exact amount in storage area. 46CFR180.78(a)(5)

On a vessel of not more than 19.8 meters (65 feet) in length carrying not more than 49 passengers, a public address system is not required if a public announcement made from operating station without amplification can be heard throughout the accommodation spaces of the vessel during normal operating conditions, to the satisfaction of the cognizant [name withheld] deck public address system is not operating as designed. 46CFR184.610(e)

As appropriate for the intended voyage, a vessel must carry adequate and up-to-date: (3) Coast Guard Light List. Onboard light list is out of date. Provide new light list within 7 days. 46CFR184.420(a)(3)

Fire hazards to be minimized. The general construction of the vessel must be such as to minimize fire hazards insofar as it is reasonable and practicable. Heating unit at operating station has exposed piping that is extremely hot. Remove plastic bin from area to minimize fire risk. Keep combustible material distant from exposed heat piping. (Operator removed plastic bin from area) 46CFR177.405(a)

On a vessel of at least 7.9 meters (26 feet) in length, a visual and audible alarm must be provided at the operating station to indicate a high water level in each of the following normally unmanned spaces: (1) A space with a through-hull fitting below the deepest load waterline, such as a lazarette. Mid stbd high bilge alarm did not operate as designed upon activation. (Operator reconnected alarm). 46CFR182.530(a)(1)

A visual indicator must be provided at the operating station to indicate when any automatic bilge pump is operating. Vessel does not have a visual indicator at operating station. Correct deficiency prior to next operating season to the satisfaction of attending marine inspector.

The Certificate of Inspection and any SOLAS Certificates must be posted under glass or other suitable transparent material, such that all pages are visible, in a conspicuous place on the vessel where observation by passengers is likely. The COI was not posted for observation by passengers. Post COI were passengers may observe certificate.

When, in accordance with § 178.210 of this chapter, a vessel must be provided with a stability letter, the stability letter must be posted under glass or other suitable transparent material, such that all pages are visible, at the operating station of the vessel. The stability letter was not posted at the operating station. Post Stability letter at the operating station.

As appropriate for the intended voyage, a vessel must carry adequate and up-to-date: Charts of large enough scale to make safe navigation possible, U.S. Coast Pilot [name withheld] List and Current tables. Current tables were only provided electronically and CG Light List was missing. Provide appropriate nautical publications for area of operation.

A power-driven vessel underway shall exhibit: (i) A masthead light forward; (ii) A second masthead light abaft of and higher than the forward one; except that a vessel of less than 50 meters in length shall not be obliged to exhibit such light but may do so; (iii) Sidelights; and (iv) A sternlight. The masthead light and anchor light arm was broken. Repair masthead light arm prior to operating at night or restricted maneuverability.

Each ship transporting more than 6 passengers for hire must carry a radio operator who meeting the following: where the station power does not exceed 250 watts carrier power or 1500 watts peak envelope power, a marine radio operator permit or higher class license. Master was unable to provide their marine radio operator permit at the time of inspection. Provide proof of valid permit. 47 CFR 80.159(e)

Unless other means are provided to ensure that a potential waste receptacle fire would be limited to the receptacle, waste receptacles must be constructed of noncombustible materials with no openings in the sides or bottom. Combustible waste receptacle being used throughout the vessel. Remove plastic waste receptacles from the vessel.

Whenever damage or deterioration to hull plating or structural members that may affect the seaworthiness of a vessel is discovered or suspected, the cognizant OCMI may conduct an internal structural examination in any affected space including fuel tanks and may require the vessel to be drydocked or taken out of service to assess the extent of damage and to effect permanent repairs. MI found 1 pit in #2 void on stbd side with was beyond allowable limits and found 4 pits in #2 void on port side beyond allowable limits. Repair pits to original condition prior to receiving dry dock credit. Due to isolated nature of pits, clad welding of pits is allowable.

Vessel's port innermost outboard engine blew its head gasket. Repair/replace engine to normal operating condition to attending Marine Inspector's satisfaction.

Vessel's starboard outboard outboard engine was not functioning properly. Repair/replace engine to normal operating condition.

An enclosed space containing machinery powered by gasoline or other fuels having a flash point of 43.3 °C (110 °F) or lower must be equipped with a flammable vapor detection device in compliance with § 182.480. The vapor detection device was not working as required. Repair/replace vapor detection device and prove proper operation to attending Marine Inspector prior to carrying passengers.

The COI must be posted in a conspicuous location and the Stability letter must be posted on the bridge. Neither the COI or Stability letter was posted. Post COI and Stability letter as required.

The middle window on Starboard side on main deck was broken. Renew window to original condition.

Non metallic hose connecting fuel fill piping to the vessels fuel tanks requires a bonding strap (wire) from the metallic section of the fill pipe to the metallic connection on the fuel tank. Found the port and starboard gasoline fuel tanks missing the bonding wire.
