PSCO found all PRVs on cargo tanks/IBS to be painted, relief valves unreadable to verify PRV setting.

The oil record book shall be completed on each occasion, on a tank-to-tank basis of appropriate, whenever any of the following operations takes place on a ship: for machinery spaces: discharge overboard or disposal of otherwise of bilge water which as accumulated in machinery spaces. ORB contained multiple entries that were incorrect. On 11FEB2020, found (.4m^3) was subtracted from inventory of bilge holding tank. The (.4m^3) was stated to be sludge from OWS and was already accounted for during OWS operation prior for (45.5m^3), rendering incorrect retension of bilge holding tank.
