74SOLAS(14)II-I/26.3.10 (17ac) Means shall be provided whereby normal operation of propulsion machinery can be sustained or restored. special consideration shall be given to the malfunctioning of the hydraulic, pneumatic or electrical means for control in main propulsion machinery. PSCO discovered the number one and number two steering gear pumps leaking hydraulic fluid from seals at approximately one drop every 8 seconds while secured and/or running. In addition the fixed storage tank required to recharge the power actuating system was 200 liters short of the minimum required amount.

Resolution: Beureau Veritas submitted Class Report attesting that "The leaking NO.1 and 2 hydraulic pumps of steering gear were repaired by ship crew and checked no leaking, the steer gear was tested in order."

PSCO discovered a fuel oil leak on the injection return line on the number one cylinder of the main engine. The leak caused an excessive amount of fuel oil to accumulate on the cylinder head causing an excessive fire risk.

Undetermined leak caused an excessive amount of fuel oil to accumulate around the high pressure pump causing an excessive fire risk.

74SOLAS(14)III/14.1 (17ac) Rescue boats shall be stowed in a state of continuous readiness for launching in not more than five minutes. PSCO discovered that rescue boat crane's hydraulic pressure unit inoperable due to a faulty solenoid valve. The rescue boat could neither be launched or recovered in five minutes. Vessel did notify Coast Guard of the problem via updated notice of arrival.

Every steam pipe and every fitting connected thereto through which steam may pass shall be designed, constructed and installed as to withstand the maximum working stresses to which it may be subjected. Steam leak was discovered on the return line to the fuel overflow tank in the engine room.
