Whenever an accident occurs to a ship or a defect is discovered either of which afect the safety of the ship or the efficiency or completeness of it's life-saving appliances or other equipment the master or owner of the shall report at the earliest opportunity to the administration the nominated surveyor or recognized organization responsible for issuing relevant certificates.
On June 19, 2025 vessel suffered an allision on the fwd stbd side frame 145 causing a 7mm hole vessel's master notified company IAW ISM but did not notify the Administration or Recognised Organization (RO). Company did not notify did not notify administration or RO either
