The company should establish procedures, plans and instructions, including checklist as appropriate for key shipboard operations concerning the safety of the personnel, ship and protection of environment. PSCO observed crew working over the side of the vessel without wearing lifejackets or following vessel safety procedures. Recommend corrective action plan from the company with concurrence of Recognized Organization.
