Resolution: Crew replaced o-ring and tightened bolts. Restarted engine and observed no leaks.

The company and the ship shall comply with the requirements of the International Safety Management Code. The company should establish procedures, plans and instructions, including checklist as appropriate for key shipboard operations concerning the safety of the personnel, ship and protection of environment. PSCO observed crew working over the side of the vessel without wearing lifejackets or following vessel safety procedures. Recommend corrective action plan from the company with concurrence of Recognized Organization. SOLAS 74(a) 2014 cons. IX/3.1 ISM Code 7

Resolution: Received, reviewed and accepted class report & proper documentation from NKK stating the deficiency was rectified. Vessel completed familiarization training for work over the side. Vessel generated work over side permit to be completed in the future.

All ships 500 gross tonnage and upwards shall have: a gyro compass heading repeater, or other means, to supply heading information visually at the emergency steering position if provided. PSCO observed 07 degrees difference between bridge gyro compass and steering gear room gyro compass repeater. SOLAS 74(a) 2014 cons V/19.2.5.2

Resolution: PSCO observed adjustment to gyro compass repeater to match bridge gyro compass.
