Due to a fire on the stern launch lifeboat, class issued an interim cert. allowing the vessel a single voyage from Portland, ME to New Orleans, LA for a new free fall lifeboat to be installed by 13 FEB 2026. 74SOLAS24 III/20.2 17ac

The purpose of this regulation is to provide means of escape so that persons onboard can safely and swiftly escape to the lifeboat and liferaft embarkation deck. Two wing nuts on the escape leading out from the forecastle deck were painted over. Crew could not escape. 74 SOLAS (2014) II-2/13.1

07 - Fire Safety › N/A - No Subsystem › Fire doors/openings in fire-resisting divisions
Issued 25 March 2019 Resolved
The fire resistance of doors shall be equivalent to that of the division in which they are fitted, this being determined IAW the fire test procedures code. Doors fitted in boundary bulkheads of machinery spaces of category 'A' shall be reasonably gas tight and self-closing. 'A-60' door to Engine Room from upper deck is not capable of closing via self-closing device nor does the door latch properly. Captain notified the company of the issue in December. Technicians were on board to repair the door, but were unsuccessful. They recommended the door be replaced, but a new door has not been ordered by company. Resolved 27 March 2019
Resolution: Vessel submitted a satisfactory class report attesting to the temporary repair of the current door and ordering of a new door.
