Maintenance, testing, and inspections of life-saving appliances shall be carried out in a manner having due regard to ensuring reliability of such appliances. PSCO observed that the hydrostatic releases expired January 2021 on 4 of the life rafts.

Resolution: All four hydrostatic relases were replaced. Verified correct installation of hyrdrostatic release, weak link, and painter.

The machinery, boilers and other pressure vessels and associated piping and fittings shall be of a design and construction adequate for the service for which they are intended. Vessel reported a MDE slowdown caused by #1 Cylinder high exhaust temperature related to a reoccurring failure to the high pressure fuel pump suction valve noted on the PSCO report dated 08SEP20. Vessel crew reported that the suction valves do not operate properly when using LSMGO due to poor lubrication. Attendance by a certified Hitachi Technician is required to assess the condition of the engine and conduct repairs as necessary. THE safety management system should include procedures ensuring that non conformities, accidents and hazardous conditions are reported to the company, investigated and analyzed. In reference to deficiency #1, the vessel SMS states in section SS-BOS that the person in charge of the ship shall fill in the report of non conformity and send it to the marine department and designated person. On 05JUL20, 08SEP20 and 09SEP20, the vessel experienced a MDE slowdown due to a malfunctioning suction valve on the high pressure fuel pump. The vessel failed to properly report and log these casualties in accordance with the vessel's SMS. The company is to complete a corrective action plan to the concurrence satisfaction of the administration that ensures the proper operation of the MDE. THE plan should also clarify and enforce proper reporting procedures for non conformities.

The condition of the ship and its equipment shall be maintained to conform with the provisions of the present regulations to ensure that the ship in all respects will remain fit to proceedto sea without danger to the ship or persons onboard. PSCO received report of a propulsion irregularity. Attended vessel and vessel personnel completed repairs of suspected failure on the #1 main engine cylinder. Failure appeared to be the fuel suction valve on the #1 fuel injection pump.

Resolved 8 September 2020
Resolution: Fault was identified to be No.1 Cylinder FO Pump Suction Valve was found stuck on account of change of lubration from Heavy Fuel Oil to LSMGO.Crew dismantled all five (5x) FO Suction Valves and proper maintenance held to his sartiasfaction.
