15 - Safety Management Systems (ISM) › N/A - No Subsystem › Shipboard operations

15106-The company should establish procedures, plans and instructions, including checklists as appropriate, for key shipboard operations concerning the safety of the personnel, ship and protection of the environment. PSCO observed that all weekly and monthly inspections for lifesaving and firefighting equipment were checked without any remarks. As indicated in deficiency No.2, all four liferafts hydrostatic releases were installed incorrectly and in deficiency No.3 11 out of 36 immersion suits were damaged. Neither Chief officer or Third Officer were able to correct the hydrostatic release arrangements without the instruction of the PSCO and could not provide manufacture instructions. Additionally, SOLAS training manual and fire fighting training manual were missing as noted in deficiency 05. Furthermore, while conducting a fire drill as stated in deficiency No.6, PSCO noticed that the SCBA bottles and firemen suits were not kept operationally ready. Due to the objective evidence in the above deficiencies, the vessel is not in substancial compliance with relevant conventions. The Captain of the Port questions the adequacy and overall implementation of the vessel's SMS under the ISM code. An internal audit is required within 90 days to determine whether the ship is operating in accordance with the ISM code.
