All foreign vessels must submit each Notice of Arrival, containing the information specified in 33CFR160.206 Table, which includes operational condition of equipment. The vessels radar is listed as equipment under 33CFR164 which is referenced in 33CFR206 Table. Vessel did not report that the vessels radar equipment is inoperable. 33CFR160.206(a) 10c

74 SOLAS 2024 III/13.4.2- Each survival craft shall be stowed with a float free arrangement complying with the requirements of 4.1.6 of the code so that each floats free automatically when the ship sinks. PSCO found that the hydrostatic release arrangement was incorrectly installed preventing automatic release.

74 SOLAS 2009 II-2/13.1.2- Escape routes shall be maintained in a safe condition, clear of obstacles. PSCO found that the Engine Room escape hatch was locked on deck preventing a means of escape from inside the Engine Room.

All ships 500 gross tonnnage and upwards shall have: rudder, propeller, thrust, pitch, and operational mode indicators, or other means to determine and display rudder angle, all to be readable from the conning position. PSCO was notified that two of the four rudder angle indicators were not operable. The remaining operable rudder angle indicator within the bridge is not viewable from the conning position. 74 SOLAS(24) V/19.2.5.4 Action 40ac

In addition to the requirments contrained elsewhere in the present regulations, ships shall be designed, constructed and maintained in compliance with the structural, mechanical and electrical requirements of a classification society which is recognized by the Administration in accordance with regulation XI/1, or with applicable national standards of the Administration which provide an equivalent level of of safety. PSCO observed a possible crack in the vessels starboard anchor well, approximately 1 meter below the bottom of the anchor. Master stated a repair was completed at last dry dock in November of 2023, however, they could not provide any class documentation for a repair or acknowledgement of the defect. Provide class documentation attesting to the structual compliance of the defect IAW classification rules. 74SOLAS(2004)II-1/3-1
