For determining the appropriate fire integrity standards to be applied to boundaries between adjacent spaces, such spaces are classified according to their fire risk as shown in categories (1) to (14) below. Where the contents and use of a space are such that there is a doubt as to its classification for the purpose of this regulation, it shall be treated as a space within the relevant category having the most stringent boundary requirements. The title of each category is intended to be typical rather than restrictive. The number in parentheses preceding each category refers to the applicable column or row in the tables. Fire doors in main vertical zone bulkheads and stairway enclosures shall satisfy the following requirements: The doors shall be self-closing and be capable of closing against an angle of inclination of up to 35 degrees opposing closure. PSCO observed fire screen #237 did not latch fully closed. Cite: 74 SOLAS 01 II-2/30.4.1 Code: 16ac

Ships shall be provided with the fire-extinguishing appliances that are readily available. PSCO discovered two fire stations missing spanner wrenches located on pport side, deck 5 IWO life boat #2, and deck 4 IWO HVAC #3. Cite: 74 SOLAS 01 II-2/10.1 Code: 16ac

Under all sailing conditions, including maneuvering ,the speed, direction of thrust and, if applicable, the pitch of the propeller shall be fully controllable from the navigational bridge. Vessel reported starboard azipod malfunctioned, was unresponsive to helm commands and operated independently of helm commands, while transiting the Detroit River. Starboard main engine was secured as the malfunctioning azipod was affecting safe navigation of the vessel. Vessel anchored at Belle Isle Anchorage for troubleshooting. Captain of the Port is requiring vessel, prior to movement, to submit causal report from Classification Society (1) identifying the contributing factors that led to the issue, (2) certifying that any necessary repairs have been satisfactorily completed, and (3) the vessel is fit for its intended route and service. SOLAS 24 II-1/49.1 Code 60ac.

Fire Doors in main vertical zone bulkheads, galley boundaries and stairway enclosures, double-leaf doors equipped with a latch necessaryto their fire integrity shall have a latch that is automatically activiated by the operation of the doors when released by the system. Double-leaf fire door FD 237 will not latch when closed. 74 SOLAS 01 II-2/30.4.12 (10C)

Every Passenger ship shall carry lifejackets for not less than 5% of the total number of persons onboard. These lifejackets shall be stowed in conspicious places on deck or at muster stations. Lifejacket stowage locker deck 4 starboard missing one adult life jacket.

Every ship of 12M or more in length overall and fixed or floating platfomrs shall display placards which notify the crew and passengers of the discharge requirements of regulation 3,4,5 and 6 of this annex. Where the ship carries passengers, placards also should be placed in prominent places where passengers are accomodated and congregate. These include cabins, all deck areas for recreational purposes. MARPOL placards were not diplayed in these areas. cite MARPOL 2017 Annex V 10.1

The activiation of any detector or manually operated call point shall initiate a visual ans audible fire signal at the control panel and indicating units. if the signals have not received attention within 2 minutes an audible alarm shall be automatically sounded throughout the crew accomodation and service spaces, control stations and machinery spaces of category A. Manual call points in aft lounge deck 2 are not operable.

Vessel must complete a satisfactory Certificate of Compliance Exam by the United State Coast Guard prior to carrying US citizens as passengers or embarking passengers in U.S. ports.

Each operation described in paragraph 2 of this regulation shall be fully recorded without delay in the Oil Record Book Part 1, so that all entries in the book appropriate to that operation are completed. ORB entry on 29 Jan 2023 was improperly denoted as a code "E" according to ORB instruction. MARPOL 2017 Annex I/17.4.

All life-saving appliances and arrangments shall comply with the applicable requirements of the Code. Except as otherwise stated, the normal equipment of every rescue boat shall consist of a searchlight with a horizontal and vertical sector of at least 6 degrees and a measured liminous intensity of 2,500 cd which can work continuously for not less than 3 hours. PSCO observed that the fuse was missing for Rescue Boat #2 search light causing the light to be inoperational. 74 SOLAS 20 III/34, LSA Code 5.1.2.2.11.

Where "A" class divisions are penetrated for the passage of electric cables, pipes, trunks, ducts, etc., or for girders, beams or other structural members, arrangments shall be made to ensure that the fire resistance is not impaired. Electrical penetration through class "A" penetration in HVAC Room #3 has improper installation of electrical cables through the marine cable transit. 74 SOLAS 01 II-2/18.1.1.

Two means of escape shall be provided from each machinery space. STBD side exit from machinery space to the bunker station improperly locked with use of a key lock from the interior preventing escape from the engine room. Emergency hatch from bunker station to galley was locked from interior, restricitng access to lifeboats or liferafts from the bunker station. 74 SOLAS 01 II-2/28.3.1.1.

All life-saving appliances and arrangements shall comply with the provisions of the code. The normal equipment of every lifeboat shall consist of a food ration as described in paragraph 4.1.5.1.18 totalling not less that 10,000 kj for each person the life boat is permitted to accomodate. PSCO found 184 food rations expired in August 2022; crew requested new rations (in accordance with SMS) in July 2022, but have not recieved the rations on board from the company. SOLAS (20) III/34 LSA CH. 4.4.4.8.12

The means provided in paragraph 2.2.3.5.2 (oil level gauge in place of sounding pipes) which are acceptable to the Administration shall be maintained in the proper condition to ensure their continued accurate functionin in service. PSCO found the port diesel day tank (8,357 gal) lower sight glass push-button stop valve to be held open in place with a wooden dowel, rendering the closing mechanism inoperable. SOLAS (04) II-2/4.2.2.3.5.3

Means shall be provided for bringing the davit-launched survival craft against the ship's side and holding them alongside so that persons can be safely embarked. PSCOs observed the 2 outboard (port) boats were not positioned adjacent to the ship with the forward end out substantially creating a large gap making it unsafe for passenger embarkation. When the crew tried to adjust the lifeboats the lifeboats turned inward making the dangerous situation even more severe. A crew member boarded the lifeboat anyway, and the lifeboat slipped 2 feet creating a hazardous situation for the crew member. The crew also stated the 2 inboard (Stbd) lifeboats are installed the same way. 74 SOLAS 01 III/11.8

Fire protection systems and firefighting systems and appliances shall be maintained ready for use. PSCO observed that ampule in the sprinkler head was not in the condition it was designed, it contained half the fluid required.

Illustration and instruction in appropriate language shall be posted at muster stations to inform passengers the method of donning lifejackets. PSCO observed no instructions for donning of lifejackets in muster stations A nor B.

Posters or signs shall be provided on or in the vicinity of survival craft and their launching controls and shall be easily seen under emergency lighting condition. PSCO observed missing emergency lights in vicinity of lifeboat launching instructions.

All lifesaving appliances and arrangements shall comply with the applicable requirements of the Code. The enclosure shall be so arranged that hand rails provide a secure handhold for persons moving about the exterior of the lifeboat, and aid embarkation and disembarkation. PSCO observed a broken bracket on the handrail on lifeboats 1 and 2.

Containers, brackets, racks, and other similar stowage locations for life-saving equipment shall be marked in acordance with the recommendations of the organization. PSCO observed lighted buoy with incorrect marking.

All life-saving appliances and arrangements shall comply with the applicable requirements of the Code. Every life boat shall consist of an operational compass which is luminous or provided with suitable means of illumination. PSCO observed that the light used to read the compass was not operating in life boats 2 and 4.

The means of escape, including stairways and exits, shall be marked by lighting or photo luminescent strip indicators placed not more than 300mm above the deck at all points of the escape route, including angles and intersections. Additionally, all escape route signs and fire equipment location markings shall be of photo luminescent material or marked by lighting. Throughout the decks accessible to passengers, there were multiple sections of photo lumonescent material not marked to identify direction of escape, fire stations, and muster stations.

An International Oil Pollution Prevention Certificate shall be issued, after an initial or renewal survey to any ships of 400 GT and above which are engaged in voyages to ports or offshore terminals under the jurisdiction of other Parties to the present Convention. PSCO observed the IOPP supplement did not have the correct throughput of the OWS.

All of vessel's statutory documentation is expired due to layup and under survey status per Bureau Veritas. This conflicts with Bahamas lay up status approval and CVC work instruction -018(1) Site: Resolution A.1052(27)

Below the bulkhead deck there shall be two means of escape, at least one of which shall be independent of watertight doors and shall be provided from each watertight compartment, or similarly restricted space or group of spaces. The workshop and HVAC room#2 does not meet this requirement.

In ships carrying more than 36 passengers, plans and booklets required by this regulation shall provided information regarding fire protection, fire detection and fire extinction based on guidelines developed by the organization. The vessels fire control plan has not been reviewed and approved by the organization.
