The international load line certificate shall not be delivered to the ship until the marks are correctly and permanently indicated on the ships sides. PSCO observed 04 painted load lines on the vessels port side. ICLL(88)II/9 10c

The company should establish procedures plans and instroctions, including checklist as appropriate for key shipboard operations, concerning the safety of the personnel, ship and protection of the environment. PSCO observed crew member using Altair 5X meter to test the oxygen level in the tank, but the readings were not accurate. Manufacturer's instroctions state that adequate oxygen must be present (> 10% O2).
