All ships of 3,000 gross tonnage and upwards shall, in addition to meeting requirements of paragraph 2.5 have a 3GHz radar or a second 9GHz radar. While inbound into the Port of San Juan, the crew reported the malfunction of the 3GHz S-Band radar and navigated into port using only the X-Band 9GHz unit. SOLAS (2014) CH V / REG 19.2.7.1

All steering gear components and rudder stock shall be of sound and reliable construction to the satisfaction of the administration. when the team tested the engine room emergency steering, it was noticed the rudder angle indicator at the control position stayed at 20 degrees port when the bridge indicator and attached mechanical pointer marked 35 degrees port. 74SOLAS(2004)29.2.1

While ships are operating within an emission control area, the sulphur content of the fuel oil used on board ships shall not exceed the following limits: 0.10% m/m on and after 1 January 2015. Due to an oversight, the vessel burned 60.5 tons of high sulphur fuel oil while in the Puerto Rico and USVI's U.S. Caribbean ECA. MARPOL VI/14.4.3.

Resolution: Vessel demarcated the US Caribbean ECA on its electronic charts. The vessel changed over to low sulphur diesel when transiting within the ECA.
