Lifebuoy on a towing vessel must: Be marked in block capital letters with the name of the vessel. Letters faded on vessel name. 46 CFR 141.360 (b)(4)

The water light must be attached by the lanyard with a corrosion-resistant clip to allow the water light to be quickly disconnected from the lifebuoy. Water light attached with non quick release shackle.

Each buoyant apparatus must have Type II retroreflective material meeting subpart 164.018 of this chapter on each side and end. Retroreflective material does not meet standard. 46 CFR 160.010-4 (p)

At least one lifebuoy must have a lifeline, secured around the body of the lifebuoy. Lifeline not properly rigged around body of lifebouys. 46 CFR 141.360 (c)(1)

Each towing vessel must be designed, constructed, and maintained so as to suitable for its intended service and route. The vessel experienced a casualty to the port Z-drive after drawing in a towing line, resulting in a potential compromise to the propulsion system. This condition impacts the vessel's ability to safely perform its towing operations and renders it not suitable for its intended service. An underwater inspection report by a qualified commercial diver, detailing the extent of damage and confirming the clearance and integrity of the Z-drive and a successful operational/sea trial, to be witnessed by Coast Guard personnel, demonstrating full functionality of the propulsion system.

Machinery and electrical systems must be designed and maintained to provide for safe operation of the towing vessel and safety of persons onboard under normal and emergency conditions. Vessel reported loss of propulsion on starboard engine. Provide technician report attesting to proper repair and operation to the satisfaction of Coast Guard prior to vessel movement. 46 CFR 143.205(a)

Machinery and electrical systems must be designed and maintained to provide for safe operation of the towing vessel and safety of persons onboard under normal and emergency conditions. Proplusion Loss on the Port engine due to fouled propeller. Marine inspectors will attend to verify operation of the shaft at the pier and provide diver report.

Portable watertight containers for pyrotechnic distress signals must be of a bright color and must be clearly marked in legible contrasting letters at least 12.7 millimeters (0.5 inches) high with “DISTRESS SIGNALS." Portable container was not labeled. Label container.

Each towing vessel must perform the applicable tests in Table 143.245(b) of this section. The tests required by this section must be recorded in accordance with part 140 of this subchapter. Pressure vessel relief valves must be tested and examined twice every 5 years, with no more than 3 years elapsing since last test. Vessel did not have record of last test of pressure vessel relief valves. Provide proof of testing/replacement and evidence that the test has been recorded.
