Each operation described in paragraph 2 of this regulation, shall be fully recorded without delay in the Oil Record Book Part 1. While reviewing the ORB, the PSCO discovered numerous inconsistencies and missed operational entries. PSCO noted Chief Engineer only recorded 01 weekly sounding in January, 01 weekly sounding in December, and 02 weekly soundings in November. [name withheld], Chief Engineer admitted to forgetting to record entries for 3.1 tanks: NO. 1 Waste Oil Settling Tank and NO. 2 Waste Oil Settling Tank in the ORB. According to daily sounding logs 0.8m3 was removed from NO. 1 Waste Oil Tank on 06DEC23, 0.95m3 oil was added to the same tank on 13DEC23, and on 30DEC23 the weekly soundings in the the ORB stated the tank had 0.2m3, but the daily sounding log recorded 1.1m3. MARPOL Annex 1/17.4, rectify prior to movement, 60.

The company should establish procedures, plans, and instructions, including checklists as appropriate, for key shipboard operations. Crew was unable to provide procedures for conducting soundings and recording weekly entries into the Oil Record Book. Chief Engineer has also consistently neglected to record those entries over the course of 3 months. Request internal audit is conducted within 90 days. SOLAS 74 (Cons) IX/2.1, ISM Code 7, rectify within 90 days.
