In meeting these requirements the company should ensure that any non-conformity is reported with its possible cause, if known. PSCO observed tank overfill-alarms active on tank (1P, 1S, 5P, 7S). This alarm is for 98%. PSCO observed tanks (1P, 1S, 5P, 7S) sounded at .93m, .96m, 1.01m, and .985m repeatedly. The overfill alarm float indicates ullage at .89m, .89m, .95m, and .93m showing a higher difference than allowed by vessel procedures +- .025m. Per ships SMS (DMP.003, 3.1.1). The captain shall report all technical failures of cargo equipment to the company. The captain stated the out of calibration overfill was not reported to company. The technical nature of the deficiency indicates a failure of the ships SMS. An internal audit, focusing on the vessels reporting procedures is due to the OCMI in 90 days. 74SOLAS(20) IX/3.1 ISM Code 10.2.2
