Combustibles and other foreign materials are not allowed within ventilation ducts. However, metal piping and electrical wiring installed in a metal protective enclosure may be installed within ventilation ducts, provided that the piping or the wiring does not interfere with the operation of fire dampers. STBD aft ER manual fire damper has wiring run through it that inerferes with the operation of the damper.

Vessel does not meet the requirements of 46CFR116.500(b). Two required means of escape must be widely separated and if possible, at opposite ends of the space to minimize the possibility of one incident blocking both escapes. Escape routes to areas of safe refuge/embarkation areas from decks 2-3 require entering the center stairwell which is open vertically from the main deck to deck five, the forward and aft stairs do not provide a continuous link between the upper and lower decks, occupants in spaces on either side of the discontinuity are entirely reliant on the central stairway to escape and would become trapped if the central stair were to be lost due to smoke/fire. Provide permanent second means of escape routes from all decks that comply with the requirements found in 46 CFR 116.500, 46 CFR 116.510 (b)(1), 46 CFR 122.606 and meets the definition of a means of escape in 46 CFR 114.400. Submit plans for modifications to the vessel’s construction to the Marine Safety Center for approval prior to modifying the general arrangement and structural fire protection of the vessel.

The vessel’s current arrangement does not meet the requirements of 46 CFR 116.500(b), which requires that each space accessible to passengers or used by crew must have two means of escape to either an area of refuge or an embarkation station. Based on limitations identified during onboard review and drill observation, the vessel’s existing layout and evacuation plan do not provide compliant secondary escape routes from several decks when the central stair tower is compromised. The operator shall submit to the local OCMI, for review and approval, a comprehensive proposal that addresses both: 1. A revised or supplemental Emergency Evacuation Plan (EEP) that includes: - A risk assessment identifying limitations in current escape routes and casualty scenarios; - Alternative procedures for evacuation to areas of refuge or embarkation stations without reliance on the central stair tower; and - Updated escape hatch and emergency exit markings in accordance with 46 CFR 122.606;

Vessel reported a failure of Port Azimuth Thruster that resulted in the vessel grounding at MM 289 of Columbia River. Prior to movement, vessel shall provide diagnosis of what caused the Port Azimuth Thruster to fail, evidence of permanent repair and inspection of structural integrity by CG Marine Inspectors.

Coaxial cable for televisions are installed through open faceplates compromising the integrity of the "B" class fire boundary between staterooms. Provide proper cable transit through the bulkhead.
