The company should ensure that: Any Non-Conformity is reported, appropriate corrective action is taken and records of these activities are maintained. Crew failed to report a ruptured pipe on the main salt water line near the bilge. The C/E acknowledged the hazard at the beginning of the voyage. Authorized a temporary repair, but did not record occurrence, order required parts, nor report to the company as required by SMS. Recommend audit of vessel’s implementation of SMS. 74SOLAS 2014 IX/3.1 ISM Code 10.2
