All life-saving appliances and arrangements shall comply
with the applicable requirements of the Code. Except as
otherwise stated, the normal equipment of every life boat
shall consist of a food ration totaling not less than 10,000 kJ for each person the lifeboat is permitted to accommodate;
kept in airtight packaging and be stored in a watertight
container. PSCO discovered 31 food rations had their
airtight packaging damaged over 04 different lifeboats. 74 SOLAS 20 III/34 LSA Code 4.4.8.12, Code 17 AC.

The condition of the ship and its equipment shall be
maintained to conform with the provisions of the present
regulations to ensure that the ship in all respects will remain fit to proceed to sea without danger to the ship or persons on board. PSCO discovered hydraulic lines on steering pump #1 and #2 were leaking. 74 SOLAS 20 I/11 (a), Code 16AC.

Fire doors in main vertical zone bulkheads, galley
boundaries and stairway enclosures, the approximate time
of closure for hinged fire doors shall be no more than 40 s and no less than 10 s from the beginning of their movement.
PSCO discovered multiple FSD (200, 143, 145, 414, 414a)
shut extremely quick, less than 5 seconds. 74 SOLAS 20 II-2/9.4.1.1.5.2 Code 10C.
07 - Fire Safety › N/A - No Subsystem › Means of escape

Resolved
Escape routes shall be maintained in a safe condition, clear of
obstacles. Fire fighting hose in vicinity of WTD 001 in the main
engine space is installed in a way that blocks the main walkway
through the space. Fire hydrant is located on one side of the walkway
and the hose is installed on the other, crossing the path of egress.
PSCO noticed throughout the engine room spaces, there are multiple
areas being used as stowage for empty cylinders, broken refrigerators,
spare parts etc that are also not secured, causing potential egress
issues. Additionally, FSD 301 was latched open and prevented door from
fwd smoking room from opening.

15 - Safety Management Systems (ISM) › N/A - No Subsystem › Maintenance of the ship and equipment

Resolved
The company and the ship shall comply with the requirements of the
International Safety Management Code. In meeting these requirements,
the company should ensure that appropriate corrective action is taken.
PSCO identified that maintenance to the HVAC ducting was conducted
improperly. The filter cover was replaced with cardboard and taped
over.
