PSCO observed multiple light fixtures in the hazardous zone on deck with missing or broken ground wires. Additionally, PSCO observed holes in electrical junction boxes located at cargo tank #3, indepedent cargo tank on starboard bow, and deck fan junction box on gas freeing fan unit #1.

The water spray system coverage was not adequate. The #1 cargo tank had no coverage, the deck tank had 50% coverage, and the port and starboard manifolds only had 66% coverage.

As identified in deficiency 02, objective evidence exists that the vessel's deck spray is not being maintained in conformity with the relevant rules (IGC Code). In review of SMS procedures relating to deck spray, on board maintenance records are contradictory to the condition of the system as observed by the [name withheld], the crew was unable to perform, follow, present, or explain any procedures regarding the use, operation, and testing of either of the vessel's two independent and mechanically different fixed gas detection systems. An internal audit and review of vessel's SMS specifically focused on the operation, maintenance, and testing of cargo safety systems, to include at a minimum fixed gas detection and deck spray, is required within 90 days.

PSCO identified flame-proof lighting with unapproved plug installed into the housing located in the SOPEP locker. The PSCO also identified an electrical panel in the motor room located next to the #3 compressor motor missing appropriate cable plug.
