Per 74SOLA(14)II-1/30.3, Where a three-phase supply is used an alarm shall be provided that will indicate failure of any one of the supply phases. Every company should develop, implement and maintain a safety management system which includes instructions and procedures to ensure safe operations of ships and protection of the environment. Vessel is provided with three-phase supply failure alarm, however safety management system dos not include instructions and procedures for testing, which may endanger the crew when testing.

The vessel reported that while enroute from Vietnam to Los Angeles, the vessel encountered bad weather causing cargo to shift in the NO. 1 cargo hold. Damage was found to frames 108 through 113 in the NO. 1 cargo hold. Temporary repairs have been completed with stiffeners. RO survey took place and permanent repairs of affected area are required to be completed prior to next docking survey on 20JUN2022.
