Master/CE was informed of condition to be corrected prior to sailing by technician. 17Jun24 Near miss found by vessel discovered that Main Engine Room CO2 room pilot bottlers had been disconnected. 13SEP24 MI discovered CO2 pilot bottles in Foam Room for Main Engine Room CO2 system were still disconnected, blinding the system from actuation. Vessel had not identified that both activation systems were blinded, leaving one main activation system inoperable and a danger to crew as CO2 would discharge directly into foam room/repair locker which could result in incapacitation , loss of vessel or death. Vessel is required to submit corrective action in 30 days that implements an SMS procedure for documentation of blinded systems, identifying how they were blinded, their return to working order and the verification of the process completed.

07109- Fixed fire extinguishing installation- The following functional requirements shall be met: fire-extinguishing appliances shall be readily available. Pilot bottles for the Main Engine room CO2 system were found disconnected in the foam room removing the ability to actuate from the remote station. In presence of [name withheld] engineer reattached pilot bottle fittings to return system to fully operational status.

01209- Manning specified by the minimum safe manning document- Whenever a vessel is deprived of service of a member of its complement, and the master or person in charge is unable to find appropriate credentialed personnel to man the vessel [...] a report of sailing short must be filed with the OCMI having cognizance for inspection in the area in which the vessel is operating or OCMI within whose jurisdiction the voyage is completed. Vessel master made sailing short notification for the position of the 1st Eng from Antwerp, Belgium to Piraeus, Greece. 46 CFR 15.725

Every boiler shall be provided with at least two means for indicating its water level, at least one of which shall be a direct reading gauge glass. Vessels gauge class was crack and provided unreliable reading. Boiler sight glass was broken and unreadable.

Every steam boiler and every unfired steam generator shall be provided with not less than two safety valves of adequate capacity. Vessels boiler had no means to verify the relief or set pressure during the relief valve test. Could not verify relief pressure when testing.

Where automated systems are provided to replace specific personnel in the control and observation of the engineering plant and spaces, or reduce overall crew requirements, the arrangements must make sure that under all sailing conditions, including maneuvering, the safety of the vessel is equal to that of the same vessel with the entire plant under fully attended direct manual supervision. Provide approved PSTP's to the satisfaction of the attending Coast Guard Marine Inspector. Complete prior to next annual inspection. 46 CFR 62.50-1

The master, owner, operator, agent, or person in charge of a vessel equipped with ballast tanks that operates in the waters of the United States must employ one of the following ballast water management methods:(1)Install and operate a ballast water management system (BWMS) that has been approved by the Coast Guard under 46 CFR part 162. Provide proper Operation, Maintenance and Safety Manual (OMSM) as shown on Coast Type Approval Certificate as well as providing correct BWMS marking (data plate) in accordance with 46 CFR 162.060-22. 33 CFR 151.2025(a)(1)

A FOOD RATION DESCRIBED IN PARAGRAPH 4.1.5.1.18 TOTALLING NOT LESS THAN 10,000 KJ FOR EACH PERSON THE LIFEBOAT IS PERMITTED TO ACCOMMODATE; THESE RATIONS SHALL BE KEPT IN AIRTIGHT PACKAGING AND BE STOWED IN A WATERTIGHT CONTAINER. PSCOS OBSERVED THAT THE STBRD LIFEBOAT FOOD RATIONS WERE NOT IN A WATERTIGHT CONTAINER.
