If electrical power is normally supplied by more than one generator, provisions shall be made, for instance by load sheding, to ensure that, in case of loss of one of these generating sets, the remaining ones are kept in operation without overload to permit propulsion and steering, and to ensure the safety of the ship. Vessel self-reported that #1 Generator began load shedding, and then the #3 generator load subsequent load shed, led to vessel experiencing a loss of propulsion. Mechanical means of failure was unable to be identified. Vessel is required to submit the following to our office. Mitigation plan in case of reoccurence to ensure safety of vessel when transiting high traffic areas, Class proposal on measures to be taken while issue is diagnosed. Techreport/Class report attesting to the identification of the resulting factors that led to the loss of propulsion. II-1/53.2.2

15108- Reports of non-conf. accidents * hazardous occurrences: The company should establish procedures for the implementation of corrective action, including measures to prevent recurrence. 14JUN24 CO2 control valves were left blinded as per yard instruction. Master/CE was informed of condition to be corrected prior to sailing by technician. 17Jun24 Near miss found by vessel discovered that Main Engine Room CO2 room pilot bottlers had been disconnected. 13SEP24 MI discovered CO2 pilot bottles in Foam Room for Main Engine Room CO2 system were still disconnected, blinding the system from actuation. Vessel had not identified that both activation systems were blinded, leaving one main activation system inoperable and a danger to crew as CO2 would discharge directly into foam room/repair locker which could result in incapacitation , loss of vessel or death. Vessel is required to submit corrective action in 30 days that implements an SMS procedure for documentation of blinded systems, identifying how they were blinded, their return to working order and the verification of the process completed.

Pilot bottles for the Main Engine room CO2 system were found disconnected in the foam room removing the ability to actuate from the remote station. In presence of [name withheld] engineer reattached pilot bottle fittings to return system to fully operational status.
