Each towing vessel must have a fire-detection system installed to detect engine room fires. The owner or managing operator must ensure the following: The system is installed, tested, and maintained in accordance with the manufacturer's design manual. During inspection, MI discovered a fault light was energized on the fire detection panel.

A power-driven vessel underway shall exhibit: (iii) operational Sidelights. MI's discovered port side running light was extinguished. Replace with in-kind equipment and make available for inspection of attending MI. Vessel is only to be operated during day light hours and clear visibility.

The results of the inspections required by 33 CFR 164.76 must be documented in the TVR, official logbook, or in accordance with the TSMS applicable to the vessel. A record of the type, size, and service of each towline, face wire, and spring line, used to make the towing vessel fast to her tow, must be available to the Coast Guard or third-party auditor for review. The following minimum information is required in the record: The dates when examinations were performed, the identification of each item of towing gear examined, and the name(s) of the person(s) conducting the examinations. During inspection, MI discovered there was no documentation on rigging/towing equipment. Three of four wire ropes on deck were missing rigging tags. The TVR was missing the above required information. MI also discovered multiple wire strands broken in wire rope that were not properly documented. Install rigging tags on missing wire ropes. Crew needs to conduct a proper inspection on all rigging equipment and log results in TVR.

Guardrails must be installed near the periphery of all decks accessible to the crew. Vsl has removed all the installed guardrails from around the main deck. Vsl must provide guardrails near the periphery of the main deck when not operating as a workboat in a worksite. 46 CFR 144.800

VHF radio must be equipped with the DSC function. Vsl has not integrated GPS into the VHF system so DSC is inop. Vsl must provide VHF radio equipped with DSC functions. 47 CFR 80.13

During underway inspection, excessive vibration and abnormal noise was identified coming from the port shaft. Identify and correct cause of vibration/noise.

Program MMSI number into radios located in the pilot house.

Replace emergency battery for fire control panel and correct fault alarm that is displayed.

Life jackets must have a light and whistle attached in accordance with 46 CFR 141.340.

Immersion suits must be stowed on the vessel for each person on board.

Provide the following placards on the vessel: Garbage management placard 46 CFR 151.59, Dischage of oil prohibited 33 CFR 155.450, Emergency duties and stations 46 CFR 140.405(c).

Provide a Health and Safety Plan in accordance with 46 CFR 140.500.

Provide an official logbook in accordance with 140.190.

Install machinery guards over rotating machinery near the main engines in the engine room.

Identify and correct low voltage warnings that continuously appear on electronic equipment in wheelhouse.

Identify and correct "loop failure" alarm on steering control panel located to the right of the helm.

Low water pressure alarm on port shaft continuously alarms when vessel is in idle or low RPMs. Correct alarm to function as intended.

Vessel is currently equipped with a Rigid buoyant apparatus. Install an Inflatable buoyant apparatus prior to operating in Cold Water (December 01).

Provide valid COD for vessel.

Provide general alarms system to meet requirements set forth in 46 CGR 143.235.

Provide means to energize fire pump at the pump.
