Vessels SMS procedure 4B.2 planned maintenance system and defect reporting delineates requirement to enter defects into "Danaos" defect reporting module. Material condition failure on EGCS, discovered with multiple soft patches as viewable from the stern not documented in "Danaos" as required.

The engine room and cargo pump room egress markings were insufficient to clearly mark escape routes, specifically pump room had contradicting placarding and engine room insufficiently identified escape routes on all decks but bottom level.
