A vessel on a rivers route must carry: Three hand red flare distress signals approved in accordance with § 160.021 in subchapter Q of this chapter; and three hand orange smoke distress signals approved in accordance with § 160.037 in subchapter Q of this chapter, or other standard specified by the Commandant. All vessel's distress signals are expired on board. 46 CFR 180.68(b)

The Coast Guard will issue a medical certificate to a mariner meeting the medical and physical standards for merchant mariners. Master must provide medical certificate per MMC requirements.

A marine employer whose vessel is involved in a casualty or incident that is, or likely to become, a serious marine incident... shall comply with the requirements of 4.06 in subchapter A of this chapter. Provide the following documents per Drug and Alcohol Program Audit. Provide required DAPI information.

Portable and semi-portable extinguishers must be inspected and maintained in accordance with NFPA 10. Service vessel's portable fire extinguishers. Vessel must complete servicing within 7 days.

The owner or managing operator shall make a vessel available for drydock examinations, internal structural examinations, and underwater surveys (UWLD) required by this section. Pontoon baffle access covers inoperable. Vessel shall make baffles ready for inspection during next drydock.

As appropriate for the intended voyage, a vessel must carry adequate and up to date: Charts of large enough scale to make safe navigation possible; U.S. Coast Pilot or similar publication; Coast Guard Light List; Tide tables; and current tables, or a river current publication issued by the U.S. Army Corps of Engineers or a river authority. Vessel must acquire all required publications within 14 days.

Sidelights mean a green light on the starboard side and a red light on the port side each showing an unbroken light over an arc of the horizon of 112.5 degrees and so fixed as to show the light from right ahead to 22.5 degrees abaft the beam on its respective side. On vessel's red and green lights, there is no arc of visibility housing or blinder installed to direct the direction in which the lights are seen.

A vessel must carry either a first-aid kit that meets the requirements in 46 CFR 199.175(b)(10) or a kit with equivalent contents and instructions. For equivalent kits, the contents must be stowed in a suitable, watertight container that is marked “First-Aid Kit”. A first-aid kit must be easily visible and readily available to the crew. First aid kit onboard had expired aspirin.

Unless other means are provided to ensure that a potential waste receptacle fire would be limited to the receptacle, waste receptacles must be constructed of noncombustible materials with no openings in the sides or bottom. Waste receptacles onboard were made of plastic construction. O/O is to remove or replace with approved receptacles.

The master or person in charge of a ship, including a drilling rig or platform, to which this section applies shall ensure that one or more placards meeting the requirements of this section are displayed in prominent locations and in sufficient numbers so that they can be read by the crew and passengers. Vessel does not have garbage handling placard.

The number and identification of the items stowed inside, and their sizes, must be marked in clearly legible letters and numbers on each container for life jackets. Remove old stenciling and place new stenciling in proper location for adult and child life jacket stowage.

Fire extinguishers installed under conditions where they are subject to dislodgement shall be installed in manufacturer's strap-type brackets specifically designed for this problem. Vessel does not have portable fire extinguishers stored in manufacturer's strap-type bracket.

When, in accordance with § 178.210 of this chapter, a vessel must be provided with a stability letter, the stability letter must be posted under glass or other suitable transparent material, such that all pages are visible, at the operating station of the vessel. If posting is impracticable, the stability letter must be kept on board in a weathertight container readily available for use by the crew and display to passengers. Vessel did not have stability letter on board.

Each number required by § 173.15 must: Be painted on or permanently attached to each side of the forward half of the vessel except as allowed by paragraph (b) or required by paragraph (c) of this section; Be in plain vertical block characters of not less than 3 inches in height; Contrast with the color of the background and be distinctly visible and legible; Have spaces or hyphens that are equal to the width of a letter other than “I” or a number other than “1” between the letter and number groupings (Example: DC 5678 EF or DC–5678–EF); and Read from left to right. Vessels starboard side stenciling of state registration number was illegible.
