Any failure of the oil filtering equipment shall be recoreded in the Oil Record Book part 1. While conducting Port State Control exam it was noticed that the oil water separators content meter is un-readable and the screen is malfunctioning. Further inquiry revealed this was first discovered by the crew on 14JUN21, but there was no entry for this failure within the Oil Record Book. MARPOL Annex I/17.5

Resolution: OCM was replaced with new and ORB entry made.
