Fuel tanks must be adequately supported and braced to prevent movement. The supports and braces must be insulated from contact with the tank surfaces with a nonabrasive and nonabsorbent material. Fuel tanks were not properly braced.

The master of the vessel shall ensure that Each EPIRB, other than an EPIRB in an inflatable liferaft, must be tested monthly, using the integrated test circuit and output indicator, to determine that it is operative. EPIRB registration expired in August 2025 and hydrostatic release expired in March 2026. Operator to remove EPIRB- not required for route.

The Coast Guard will issue a medical certificate to a mariner meeting the medical and physical standards for merchant mariners. The medical certificate will be issued for various periods of time based upon the endorsements the mariner holds. The Coast Guard will review all information provided and will determine. The medical certification for Captain was expired.

A vessel must carry either a first-aid kit that meets the requirements in 46 CFR 199.175(b)(10) or a kit with equivalent contents and instructions. For equivalent kits, the contents must be stowed in a suitable, watertight container that is marked “First-Aid Kit”. First Aid kit on board had significant water intrusion.

Grab line. The grab line shall be 3 8-inch diameter polyethylene, polypropylene, or other suitable buoyant type synthetic material having a minimum breaking strength of 1,350 pounds. Vessels grab lines for two life buoys were deteriorated.

Each life jacket must be marked with Type I retroreflective material approved in 46 CFR 164.018 or other standard specified by the Commandant. 16 adult and 5 child size life jackets required reapplication of retro-tape. Vessel restricted to carrying 10 adults and 3 child passengers. //46 CFR 185.604(h)//

Marine employers shall establish programs for the chemical testing for dangerous drugs on a random basis of crew members. There's currently no program in place. Provide proof of entry into a program and proof that all employees are enrolled prior to carrying passengers. 46 CFR 16.230(a)

The employer shall provide an Employee Assistance Program (EAP) for all crewmembers. EAP education program: Each EAP education program must include at least the following elements: display and distribution of informational material; display and distribution of a community service hot-line telephone number for crewmember assistance, and display and distribution of the employer's policy regarding drug and alcohol use in the workplace. (b) EAP training program: An EAP training program must be conducted for the employer's crewmembers and supervisory personnel. The training program must include at least the following elements: the effects and consequences of drug and alcohol use on personal health, safety, and work environment; the manifestations and behavioral cues that may indicate drug use. There's no program in place. Provide proof of entry into a program and the required training. 46 CFR 16.401(a)&(b)

Each disposable hydrostatic release unit must be marked with an expiration date of two years after the date on which the unit is installed. EPIRB hydrostatic release unit is expired. Replace unit with current one. Operations restricted; no operations beyond the demarcation line. 46 CFR 185.740(b)

A vessel on an oceans, coastwise, limited coastwise, or Great Lakes route must carry six hand red flare distress signals and six hand orange smoke distress signals or 12 red distress signals. Vessel currently has 2 non-expired red distress signals on board. Provide an additional 10 distress signals prior to carrying passengers. 46 CFR 180.68(a)

Each battery must be located as high above the bilge as practicable, secured to protect against shifting with the roll and pitch of the vessel, and free from exposure to water splash or spray. Battery is not enclosed to protect from exposure from water. Provide proper enclosure for battery. 46 CFR 183.350(b)

As appropriate for the intended voyage, a vessel must carry adequate and up-to-date: U.S. Coast Pilot [name withheld] and Navigation Rules. Publications not provided. Provide publications for vessel's route. 46 CFR 184.420

The master and crew of a vessel will be familiar with the content of and have mounted at the operating station, emergency instructions containing the actions to be taken in the event of fire, heavy weather, or man overboard conditions. Post emergency instructions. 46 CFR 185.510

A vessel must carry either a first-aid kit that meets the requirements in 46 CFR 199.175(b)(10) or a kit with equivalent contents and instructions. First aid kit on board is expired. Provide a current kit. 46 CFR 184.710

Remote fuel shutoff stations must be marked in clearly legible letters at least 25 millimeters (1 inch) high indicating purpose of the valve and direction of operation. Replace labels that are no longer legible with new labels. 46 CFR 185.608

Crew training shall be logged or otherwise documented for review by the Coast Guard upon request. Provide drill logs. 46 CFR 185.420(d)

Your vessel must undergo an annual inspection within the 3 months before or after each anniversary date. Vessel's anniversary date is November 8th. Vessel did not complete annual inspection within 3 months of the anniversary date. 46 CFR 176.500(b)(1)

Each vessel must be equipped with at least two operable portable battery lights. MI observed only one operable portable light to be operational onboard. 46 CFR 183.430
