All vessels must have operable navigation lights in accordance with the Navigation Rules. Stern light inoperable. Provide navigation lights that are in compliance with the Navigation Rules. Vessel restricted to operations only during daylight hours until stern light renewed. 46 CFR 183.420

An exposed hazard, such as gears or rotating machinery, must be properly protected by a cover, guard, or rail. Port and Starboard engines require guards over exposed engine drive belts. 46 CFR 177.960

Repairs or alterations to the hull, machinery, or equipment that affect the safety of the vessel must not be made without the approval of the cognizant OCMI. Vessel required to modify BBQ to include jettison device per Sector Honolulu work instruction 46, at next scheduled drydock December 2025. Meanwhile vessel will follow all other action steps required in part 5 of work instruction 46 and also all "best practice" details stated in company Letter/SOP as a means of mitigating fire risk. 46 CFR 176.700 OCT2025: Update will be removed when completed. Revisions to WI46 in progress and vessel has been informed they may continue use of grill as stipulated above until WI46 revision is approved at which time the vessel must comply or seek additional special consideration.

A vessel on which a pre-engineered fixed gas fire extinguishing system is installed mush have the following equipment at the operating station: a light to indicate discharge, and and audible alarm that sounds upon discharge. Port visual alarm at operating station not operational and port audible alarm at operating station not operational. Provide both visual and audible alarms for port pre-engineered fixed gas extinguisher at operating station. 46 CF 181.420(b)

All fiber reinforced plastic vessels constructed with general purpose resins must be fitted with a smoke activated fire detection system of an approved type installed in accordance with 76.27 in subchapter H of this chapter in accommodation spaces and service spaces. Vessel does not a have smoke activated fire detection system for required spaces. Provide smoke activated fire detection system and install to the satisfaction of the attending marine inspector. 46 CFR 177.410(c)(3)(ii)

Provision must be made for closing all supply duct cowls or scoops and exhaust duct discharge openings for a space protected by a fixed gas extinguishing system. All closure devices must be readily available and mounted in the vicinity of the vent. Ventilation duct opening in engine rooms was stuck open when closure activated. Conduct maintenance to ensure vent closures close at all times when operated. 46 CFR 182.465(h)

The employer shall provide an Employee Assistance Program for all crew members that includes both education and training on the effects and consequences of drug and alcohol use. Supervisory personnel must receive at least 60 minutes of training and general drug awareness training for crew. Documentation of training could not be provided. Provide satisfactory evidence of an Employee Assistance Program training for supervisors and crew. 46 CFR 16.401

Each ring life buoy on a vessel must be approved in accordance with 160.050 in subchapter Q. Ring buoy is damaged and cracked. Replace with usable ring buoy. 46 CFR 180.70(b)(1)

Ring life buoy must be in serviceable condition with 01 floating wafer light with 3-6 feet of line attached to ring buoy by corrosion resistant clip. Ring life buoy sun damaged, water light not operational, and water light line not attached. Provide serviceable ring life buoy, water light, and 3-6 foot line for light. 46 CFR 180.70

Vessel shall have 06 hand red flares, and 06 hand orange smoke flares. Missing compliment of flares. Provide flares. 46 CFR 180.68

A visual indicator must be provided at the operating station to indicate when any automatic bilge pump is operating. Stbd mid auto bilge pump indicator not operational. Provide operational auto bilge indicator. 46 CFR 182.530

04 UL 217 independent modular smoke detectors must be mounted and operational in both heads and storage spaces. 02 smoke detectors not operational. Provide operational UL 217 smoke detectors. 46 CFR 181.450 Special Consideration

Vessel does not currently meet the requirements of Work Instruction 31. Mandated crew posted on COI may not enter the water or act as lifeguard until policy and procedure provided. Provide written procedure. Endorsement will be removed from COI if not completed in 30 days. Work Instruction 31

Vessel Preventative Maintenance Plan (PMP) on vessel but inspection regime not being followed. Update Sail Rigging Preventative Maintenance Plan (PMP) with all current configurations. In PMP provide description of rig, sail plan, full rigging inventory, and inspection regime. Ensure to log all inspections in accordance with vessel PMP. Keep copy of PMP on board vessel at all times and submitted revised version of PMP to [name withheld] 02-16

Furler required to be disassembled for wire, fitting inspection every 6 months in accordance to PMP. No log to show when furler was last disassembled in the past year. Disassemble furler and conduct inspection on rigging components. Company owned Sail Rigging PMP

Update Company Drug Policy/Paperwork to reflect current changes to DAPI program (i.e., Provide EAP information; SAP information; Hot-line information, etc).

The Designated Employee Representative (DER) should be executing the notification process to employees for completion of DOT Random Drug Tests.

Establish record system as discussed during DAPI Audit. Provide MIS reports for 2016 and 2018. Ensure record system is secured and organized in DER office.

Provide Letter of Regulatory Compliance (LORC) for Reliable Drug Testing Services Consortium.

An EAP training program must be conducted for the employer's crewmembers and supervisory personnel. The training program must include at least the following elements: the effects and consequences of drug and alcohol use on personal health, safety, and work environment; the manifestations and behavioral cues that may indicate drug and alcohol use and abuse; and documentation of training given to crewmembers andthe employer's supervisory personnel. Supervisory personnel must receive at least 60 minutes of training. Provide proof of completed EAP training for all personnel.

Vessel shall be equipped with life floats meeting 46 CFR 160.027 in accordance with 46 CFR 180.200. Vessel is currently outfitted with buoyant apparatus that meet 46 CFR 160.010.

Pre-engineered fixed gas fire extinguishing systems must be capable of manual actuation from outside the space in accordance with 46 CFR 181.420. Safety pins were inserted in installed extinguishing system preventing manual activation.

General purpose resin may be used if a smoke activated fire detection system of an approved type, installed in accordance with 46 CFR 76.27,is fitted in all accommodation spaces. No centralized smoke detection system was provided for port and stbd washroom/toilet space.

Vessel must comply with applicable requirements of FCC including installation certificates in accordance with 46 CFR 184.502. Radio Safety Certificate was not provided.

Fuses must be cartridge type and must bye listed by UL or other laboratory recognized by the Commandant in accordance with 46 CFR 183.380(l). Blade type fuses are used in electrical systems. To be completed at next drydock.

The opening in the hull for submersible bilge pump must be as high above the waterline as possible in accordance with 46 CFR 182.520(e)(6). Bilge pump overboard discharges are not as high above the waterline as possible. To be completed at next drydock.

Vessel must have permanent draft or loading marks in accordance with 46 CFR 185.602. Provide loading or draft marks. To be completed at next drydock.

Provide proof of Masters License.

Provide Alcohol/Drug testing supplies.

Prove crew is in compliance with DAPI Requirements.

Conduct drills as required by the attending Marine Inspector during new to zone exam.

Prove compliance with NVIC 02-16 INSPECTION GUI9ADANCE FOR SAIL RIGGING AND MASTS ON INSPECTED SAILING VESSELS to the satisfaction of the attending Marine Inspector.

Provide Certificate of Documentation.

Provide FCC radio Operator Certificate as required by 47 CFR 13.
