Fixed gas fire extinguishing system design and installation must be in accordance with the Marine Design, Installation, Operation, and Maintenance Manual approved for the system by the Commandant. Vessel's STBD pilot bottle manual control box was loose. Vessel had a loose cap and a loose union on the pull station piping. Fixed fire system needs to be repaired and inspected. 46 CFR 181.410 (a)(3)

The master shall conduct sufficient drills and give sufficient instructions to make sure that all crew members are familiar with their duties during emergencies that necessitate abandoning ship or the recovery of persons who have fallen overboard. Upon arrival at new OCMI port (New York) prior to carrying of passengers company is to assign captain and crew to vessel, conduct appropriate familiarization training, and perform fire and abandon ship drills for local OCMI marine inspector.
