Fire doors in main vertical zone bulkheads, galley boundaries and stairway encloseures, the approximate time of closure for hinged fire doors shall be no more than 40 s and no less than 10 s from the beginning of their movement. PSCO observed Fire Door 06.2.24 was unable to fully latch closed. 74SOLAS20 II-2/9.4.1.1.5.2 10c

Fire doors in main vertical zone bulkheads, galley boundaries and stairway enclosures, double-leaf doors equipped with a latch necessary for their fire integrity shall have a latch that is automatically activated by the operation of the doors when released by the system. PSCO's observed double-leaf door 06.2.09 in stairway enclosure not close in sequence, preventing the door from closing completely. 74 SOLAS 20 II-2/9.4.1.1.5.12

Escape doors from public spaces that are normally latched shall be fitted with a means of quick release. Such means shall consist of a door-latching mechanism incorporating a device that releases the latch upon the application of a force in the direction of escape flow... .3 not be equipped with any locking device, set screw or other arrangement that prevents the release of the latch when the pressure is applied to the releasing device. PSCO's observed two locations where the doors along a passenger escape paths were capable of being locked by either locking latches and/or turn locks. Location 1) entrance/exit to Vista Lounge both port and starboard side. Location 2) Entrance/exit by the shops near FSD 7.3.05 and FSD 7.3.08. 74 SOLAS 20 II-2/13.3.2.6.2

PSCO observed a cat 4/7 space, directly behind the Vista Lounge in which each escape route could be locked from outside the space rendering both means of escape ineffective. 74 SOLAS 20 II-2/13.3.2.2

An owner or operator required to have security plan under parts 104, 105, or 106 of this sub chapter shall, without delay, report breaches of security to the National Response Center via one of the means listed in paragraph (a) of this section. Passenger Vessel Enchanted Princess found a breach of security on board and did not report to the National Response center without delay. Vessel delayed communication by three hours while they conducted further investigation.

THE DESIGN SHALL HAVE REGARD TO PATERIALS USED IN CONSTRUCTION, THE PURPOSE FOR WHICH THE EQUIPMENT IS INTENDED, THE WORKING CONDITIONS TO WHICH IT WILL BE SUBJECTED AND THE ENVIRONMENTAL CONDITIONS ONBOARD.
