A suitable hull strainer must be installed in the circulating raw water intake line of an engine cooling water system. The simplex hull strainer, located in the port side engine compartment, is not in suitable condition. Hull strainer is leaking sea water into engine compartment. Repair or replace hull strainer to the satisfaction of the attending marine inspector. 46 CFR 182.420(a)(2)

A vessel with installed toilet facilities must have a marine sanitation device that complies with 33 CFR part 159. When operating a vessel on a body of water where the discharge of treated or untreated sewage is prohibited by the Environmental Protection Agency under 40 CFR 140.3 or 140.4, the operator must secure each Type I or Type II device in a manner which prevents discharge of treated or untreated sewage. Acceptable methods of securing the device include—(1) Closing the sea cock and removing the handle;(2) Padlocking the sea cock in the closed position;(3) Using a non-releasable wire-tie to hold the sea cock in the closed position; or(4) Locking the door to the space enclosing the toilets with a padlock or door handle key lock. The overboard discharge valve on the marine sanitation device on board is not secured by any means listed above. Secure the overboard discharge valve to the satisfaction of the attending marine inspector. 46 CFR 184.704 33 CFR 159.7 (b)

Connections must be made to battery terminals with permanent type connectors. Spring clips or other temporary type clamps are prohibited. Engine start battery for port engine has wing nuts. Replace wing nuts to permanent type connectors to the satisfaction of the attending marine inspector. 46 CFR 183.350 (d)

Strong and effective rudder stops must be provided to prevent jamming and damage to the rudder and its fittings. Steering gear in starboard compartment does not have a rudder stop on the outboard side. Provide suitable rudder stop on the outboard side of the starboard steering gear to the satisfaction of the attending marine inspector. 46 CFR 182.610(e)

A vessel must carry either a first-aid kit that meets the requirements in 46 CFR 199.175(b)(10) or a kit with equivalent contents and instructions. Vessel has expired aspirin. Replace with required 48 doses to the satisfaction of the attending marine inspector. 46 CFR 184.710

An EAP training program must be conducted for the employer's crew members and supervisory personnel. The training program must include at least the following elements: the effects and consequences of drug and alcohol use on personal health, safety, and work environment; the manifestations and behavioral cues that may indicate drug and alcohol use and abuse; and documentation of training given to crew members and the employer's supervisory personnel. Supervisory personnel must receive at least 60 minutes of training. No evidence provided of supervisor and crew drug awareness training. Provide documentation of the training described above to the satisfaction of the attending marine inspector. 46 CFR 16.113(a)

Abandon ship and man overboard drills and training shall be logged or otherwise documented for review by the Coast Guard upon request. No evidence of drill log book on board. Create log book and provide documentation to the satisfaction of the attending marine inspector. 46 CFR 185.520(f)

The discharge ends of fuel tank vent pipes must be fitted with removable flame screens. The flame screens must consist of a single screen of corrosion resistant wire of at least 30 x 30 mesh. The fuel vent on the starboard side of the vessel is corroded. Repair or replace vent to the satisfaction of the attending marine inspector. 46 CFR 182.450(e)

Except as noted in § 176.114 of this part, each vessel inspected and certificated under the provisions of this sub chapter must, when any passengers are aboard during the tenure of the certificate, be in full compliance with the terms of the certificate. Vessel Certificate of Inspection (COI) has Sector Honolulu's Work Instruction 31 (WI-31) in the COI's Route Permitted and Conditions of Operation. The required WI-31 medical oxygen cylinder is overdue for its five-year hydrostatic test by one year and two months in accordance with 49 CFR 180.209 Table 1. Conduct the required hydrostatic test on oxygen cylinder or replace with a compliant oxygen cylinder to the satisfaction of the attending marine inspector. 46 CFR 176.100(b)

Each survival craft, rescue boat, and launching appliance on a vessel must be inspected monthly, using the manufacturer's instructions to make sure it is complete and in good order. One life float on board the vessel vinyl covering is excessively cracked exposing the polystyrene core. Repair with an approved vinyl repair kit or replace life float to the satisfaction of the attending marine inspector. 46 CFR 185.722

Rails or equivalent protection must be installed near the periphery of all decks of a vessel accessible to passengers or crew. Deck rails must include a top rail with the minimum height required, and lower courses or equivalent protection as required. The vessel's forward bow from port to starboard does not have deck rails or approved equivalent protection. Submit a written policy addressing how the vessel will implement physical or operational measures to a level that is considered an equivalent protection to railings to be approved by the OCMI. 46 CFR 177.900(a), (f)

The cognizant OCMI may require that a vessel and its equipment undergo any additional test or inspection deemed reasonable and necessary to determine that the vessel and its equipment are suitable for the service in which they are to be employed. Vessel has not completed sea trials under sail following 6yr mast inspection. Vessel is restricted from operating under sail until sea trials are completed under sail.

The cognizant OCMI may require that a vessel and its equipment undergo any additional test or inspection deemed reasonable and necessary to determine that the vessel and its equipment are suitable for the service in which they are to be employed. The sail equipment preventive maintenance plan (PMP) was not carried out by the crew. Provide a third party sail rigging survey report and record PMP carried out by the owner or operators to the satisfaction of the attending Marine Inspector. 46 CFR 176.840

Each life jacket must be marked with Type I retroreflective material approved in 46 CFR 164.018 or other standard specified by the Commandant. 10 child sized life jackets were missing the functional retroreflective material. Repair the life jackets to satisfaction of the attending Maine Inspector. Limit the passengers allowable onboard to adults only until the life jackets are repaired or replaced. 46 CFR 185.604(h)

Each buoyant apparatus must have Type II retroreflective material meeting subpart 164.018 of this chapter on each side and end. The lifefloat was missing the functional retroreflective material. Repair the lifefloat to satisfaction of the attending Maine Inspector. 46 CFR 180.200(2) 46 CFR 160.010-4(p)

Each child size life jacket must be stowed in a location that is appropriately marked and separated from adult life jackets so the child size life jackets are not mistaken for adult life jackets. The storage spaces for both adult and child size life jackets are not properly marked. Mark the life jacket storage spaces to the satisfaction of the attending Marine Inspector. 46 CFR 180.78(a)(5)

Man overboard and fire fighting drills and training shall be logged or otherwise documented for review by the Coast Guard upon request. Drill entries were missing on the log. Crew conducted drills and made entries during the inspection. 46 CFR 185.520(f) 46 CFR 185.524(d)

The master shall ensure that the pollution prevention placards are displayed in prominent locations and in sufficient numbers so that they can be read by the crew and passengers. The garbage pollution prevention placard was not displayed at location readily accessible to the passengers. Placard displayed on the passenger space. 46 CFR 184.702 33 CFR 151.59(a)

Where a flexible vent pipe section is necessary, suitable flexible tubing or hose having high resistance to salt water, petroleum oils, heat and vibration. Fuel tank vent hoses in both port and starboard engine compartments, have Type B2 rated hose. Replace tank vent hoses with Type A1 or A2 hose. 46CFR182.450(g)

Each item of lifesaving equipment determined by the marine inspector to not be in serviceable condition must be repaired or replaced. Cracks found in top of life float. Repair cracks found in life float. 46 CFR 176.808(b)

Deck rails must be designed and constructed to withstand a point load of 200 pounds applied at any point in any direction. Fractured welds on deck effecting strength of railings. Repair all fractured welds found. 46 CFR 177.900 (b)

Pipes used for wet exhaust lines must be schedule 80 or corrosion-resistant material. STBD engine exhaust piping require replacement due to running rust and excessive corrosion.

Each buoyant apparatus must have Type II retroreflective material meeting subpart 164.018 of this chapter on each side and end. Life float requires retro tape replacement.

Each ship transporting more that six passengers for hire equipped with a radiotelephone station must carry a radio operator who: holds a marine radio operator permit or higher class license. Master's radio operator permit not onboard. 47 CFR 80.158 (e)

All machinery and equipment must be installed and maintained in such a manner as to afford adequate protection from causing fire, explosion, machinery failure, and personnel injury. Vessel's electronic throttle control system failed, resulting in a loss of propulsion. The electronic throttle control shall be repaired or replaced to the satisfaction of the attending USCG Marine Inspector and the proper operation of the vessel's propulsion system shall be demonstrated to the attending USCG Marine Inspector. 46 CFR 182.100

The owner shall be prepared to conduct tests and have the vessel ready for inspection of the following: On a sailing vessel spars, standing rigging, running rigging,blocks, fittings, and sails. The vessel did not have onboard the preventative maintenance plan available for review. Provide a copy of the vessel's current PMP for review IAW NVIC 02-16. 46CFR176.802(a)(3)

The owner shall be prepared to conduct tests of the following: Operational test of the means provided for pumping the bilges. The crew onboard was unfamiliar with operation of the fixed bilge system. Emergency procedures were updated and crew demonstrated operation to the satisfaction of the attending Marine Inspector. 46CFR176.804(h)

On a vessel of not more than 19.8 meters (65 feet) in length carrying more than 49 passengers, and on a vessel of more than 19.8 meters (65 feet) in length, the minimum capacity of the fire pump must be 189 liters (50 gallons) per minute at a pressure of not less than 414 kPa (60 psi) at the pump outlet. The pump outlet must be fitted with a pressure gauge. During operational test of fire pump, pressure gauge at outlet was inoperative. 46 CFR 181.300(b)

Upon arrival to operating [name withheld], vessel must have new to zone conducted. 46 CFR 176.404
