74 SOLAS (09) II-2/4.1.3 TO PREVENT THE IGNITABLILITY OF COMBUSTIBLE MATERIALS SHALL BE RESTRICTED. PSCO OBSERVED FUEL OIL LEAK ON #2 CYLINDER HEAD ON THE M/E.

The purpose of this regulation is to prevent the ignition of combustible materials or flammable liquids. Crew reported a fuel leak coming from heavy fuel oil No.3 transfer pipe in the space located within the #9 void space double bottom. Permanent repair needed to the satisfaction of Class Society.

Electrical installations shall be such that the safety of passengers, crew, and ship from electrical hazards will be ensured. Found dead end cables from previously removed light fixture in engineers workshop; breaker found closed & wires energized, but taped over.

The machinery... associated piping systems and fittings shall be of a design & construction adequate for the service in which they are intended. On the coolant water (seawater) inlet line, a temperature probe providing seawater inlet temperature is removed; a blanked cap is in place on the line.

The condition of the ship & its equipment shall be maintained. All main engine injector sight glasses are cracked or non-visible for intended use. Crew has previously requisitioned parts for replacement.

Electrical installations shall be such that all electrical auxiliary services necessary for maintaining the ship in normal operational and habitable conditions will be ensured. PSCO observed tape over the RESET button on #1 generator's main breaker; the breaker's normal operating capabilities are in question. Vessel shall not utilize the #1 generator while in US waters.

The Administration may permit the use of oil level gauges with flat glasses and self closing valves between the gauges and fuel tanks. Vessel's MGO Daily tank upper gauge glass valve found leaking a steady stream of fuel. Upper and lower gauge valves on MGO Daily, Settling, Aux MGO, ME Lube Oil, Aux Lube Oil and Lube Oil Storage tanks found frozen in the open position.

Maintenance, testing and inspections of lifesaving appliances shall be carried out in a manner having due regard to ensuring reliability of such appliances. Port and STBD lifeboats found with fiberglass repairs on transom inner hull seam, stbd sides. STBD lifeboat found with repair on port quarter, inside and outside of boat in way of rub rail.

The company should establish procedures, plans and instructions, including checklists as appropriate, for key shipboard operations concerning the safety of personnel, the ship and protection of the environment. Vessel's container with forward life jackets and immersion suits found pad locked closed. Lock was placed on 12MAY24. No procedure exists to make sure the lock is removed prior to departure from port.

Before the ship leaves port and at all times during the voyage, all life saving appliances shall be in working order and ready for immediate use. As indicated in deficiency #3, Lifejacket and Immersion Suit container was locked. Crew removed the pad lock.

Jacketed piping systems shall include a means for collection of leakages and arrangements shall be provided with an alarm in case of a fuel line failure. Main engine recirculation piping covering covered in oil.

Liferafts intended for throw-overboard launching shall be so stowed as to be readily transferable for launching on either side. Forward liferaft on starboard side not redily transferable from starboard to port.

The condition of the ship and its equipment shall be maintained to conform with the provisions of the present regulations to ensure that the ship in all respects will remain fit to proceed to sea without danger to the ship or persons on board. Vessel experienced a loss of propulsion while mooring at Freeport, TX. Vessel received a failure of EGS system, reset the system, and regained power. Vessel reps do not know the cause of the failure. 74SOLAS2014 I/11(a)

11129- Life jackets shall be so placed as to be readily accessible. PSCO observed storage of lifejackets at the fwd six man liferaft were secured with a pad lock. 74 SOLAS (14) III/7.2.2

11129- Immersion suits shall be placed as to be readily accessible. PSCO observed storage of immersion suits up fwd ajacent to the 6 man liferaft secured with a padlock. 74 SOLAS (14) III/32.3.4

07199- Means shall be provided to control leaks of flammable liquids. PSCO observed fuel leaks on the M/E No. 1 cylinder head and the M/E No.5 and 6 fuel pumps with oil accumulation at the base of the M/E and in the oil drainage resevoir.In addition a fuel line in the vicinity of the No.1 generator that leads to the service tank in the purifier room had oil soaked lagging. The HFO sample line on the cargo deck No.1 was found with leaking oil accumulated around it. 74 SOLAS (14) II-2/4.1.1

06101- Cargo shall be loaded, stowed and secured in accordance with the cargo securing manual. In accordance with the vessel's cargo securing manual, lashings should only be attached to the securing points provided for that purpose. PSCO observed vehicles on deck 13 secured to guard rails stanchions. 74 SOLAS (14) VII/5

15108- The safety management system should include procedures ensuring non-conformities. Accidents and hazardous situations are reported to the company. PSCO observed a structural dent in the vessel's hull. the master was unable to provide documentation of the hull's condition being reported to the company as required by the vessel's safety management system. 74SOLAS(14) IX/31 & ISM Code 9.1

07103-Structural fire protection, including fire resisting divisions shall be kept in good order. PSCO observed damaged insulation on A-Class bulkhead on cargo deck 2 and boatsons stores. 74SOLAS(14) II-2/14.2.1.1.1

09209-After any survey of the ship has been completed no change shall be made. Bonding strips on 3 pipe connections for the fuel oil lines on deck 1 were disconnected. 74SOLAS(14) I/11(b)

07122-Escape route signs and fire equipment location morkings shall be photoluminescent material. PSCO observed multiple exit and fire equipment signs that were not photoluminescent when tested. 74SOLAS(14) II-2/13.3.2.5.1

15110-Every company should develop, implement, and maintain a safety management system. The vessel's safety management manual requires the company to update the manual when new equipment is installed. The vessel had a ballast water treatment system instaled in July 2020 and did not update stafety management procedures or the ballast management plan. 74SOLAS(14) IX/3.1 & ISM Code 1.4

15109-The company should establish procedures to ensure that the ship is maintained in conformity with the provisions of relevant rules and regulations and with any additional requirements which may be established by the company. PSCO observed multiple issues regarding the safe management of the vessel. Vessel fasiled to make a report required by the safety management system regarding the inset in the hull. Life saving equipment required to be readily available was locked up while sailing international. Containers and vehicles were not secured in accordance with the cargo securing manual. Due to the objective evidence in the above deficiencies the vessel is not in substantial compliance with relevant conventions.The Captain of the Port questions the adequacy and overall implementation of the vessels safety management system under the ISM Code. An internal audit is required within 30 days to determine whether the ship is operating in accordance with the ISM Code. Provide a copy of the audit report to the USCG when completed. 74SOLAS(14) IX/3.1 & ISM Code 10.1

99101-Any vessel equipped with ballast water tanks that operates in th U.S. must rinse anchors and anchor chains when the anchor is retrieved to remove organisms and sediments at the place of origin. PSCO observed sediment on port anchor and chain, vessel last anchored in Freeport, TX. 33 CFR 151.2050
