Vessel's MGO Daily tank upper gauge glass valve found leaking a steady stream of fuel. Upper and lower gauge valves on MGO Daily, Settling, Aux MGO, ME Lube Oil, Aux Lube Oil and Lube Oil Storage tanks found frozen in the open position.

07199- Means shall be provided to control leaks of flammable liquids. PSCO observed fuel leaks on the M/E No. 1 cylinder head and the M/E No.5 and 6 fuel pumps with oil accumulation at the base of the M/E and in the oil drainage resevoir.In addition a fuel line in the vicinity of the No.1 generator that leads to the service tank in the purifier room had oil soaked lagging. The HFO sample line on the cargo deck No.1 was found with leaking oil accumulated around it. 74 SOLAS (14) II-2/4.1.1

15109-The company should establish procedures to ensure that the ship is maintained in conformity with the provisions of relevant rules and regulations and with any additional requirements which may be established by the company. PSCO observed multiple issues regarding the safe management of the vessel. Vessel fasiled to make a report required by the safety management system regarding the inset in the hull. Life saving equipment required to be readily available was locked up while sailing international. Containers and vehicles were not secured in accordance with the cargo securing manual. Due to the objective evidence in the above deficiencies the vessel is not in substantial compliance with relevant conventions.The Captain of the Port questions the adequacy and overall implementation of the vessels safety management system under the ISM Code. An internal audit is required within 30 days to determine whether the ship is operating in accordance with the ISM Code. Provide a copy of the audit report to the USCG when completed. 74SOLAS(14) IX/3.1 & ISM Code 10.1
