Crew was unable to demonstrate adequate knowledge and familiarization of their specific duties and ship arrangements in regards to the fuel handling and delivery system as required per IGF Code. STCW 1/14.1.5 (Code 17ac)

Crew was unable to demonstrate to the PSCO satisfactory operation or testing of emergency shutdowns, alarms, and equipment as a part of the fuel handling and delivery system as required per IGF Code. Resolution: PSCO observed satisfactory operation of tank high level alarms, emergency shutdowns, flame/fire detection, and gas detection during inspection.

Drills and emergency exercises on board shall be conducted at regular intervals. The PSCO identified multiple emergency situations/drills that shall be carried out every 3 months in accordance with their Safety Management System, including MEOH pipe burst and leakage, gas detection, accidental MEOH spill in the engine room, and gas detection in double wall piping. Resolution: The PSCO received objective evidence that a satisfactory MEOH drill was conducted. Additionally, the SMS was updated to reflect additional requirements for these drills to be completed on a quarterly basis.

Maintenance procedures and information for all gas related installations shall be available on board. The vessel’s Safety Management System (SMS) required spray test gas (vapor) appropriate for methanol to test sensors in installed double wall piping. The PSCO was not provided any proof that the test gas was onboard or that testing would conducted in accordance with the SMS and maintenance plan. SOLAS (20) II-1/57, IGF 18.2.2 (Code 17ac)

Resolution: PSCO received objective evidence that test gas was present on board and verified during the inspection the functionality of methanol sensors throughout the fuel system.
