FSS Code 15/2.2.1.2.5 The [IG] system shall be capable of delivering inert gas with an oxygen content of not more than 5% by volume.
Vessel found with expired nitrogen span gas bottle connected to IG system and being used to calibrate the IG system. Nitrogen span gas expired on 5/20/2022. Last discharge (and use of IG system) was 5/28/2022 as verified by E/R log book and crew admission. To wit, expired span gas was used to calibrate the system relied upon to keep vessel cargo tanks in a non-explosive atmosphere condition.

Every company should develop, implement and maintain a SMS which includes... instructions and procedures to ensure safe operation of ships and protection of the environment. On 5/28/2022 vessel discharged cargo and used IG system to maintain inert atmosphere in cargo tanks. Nitrogen span gas used for calibration of IG system expired on 5/20/2022. To wit, vessel violated SMS Procedure "SOP-MEV-01-01 Inert Gas System" in that vessel did not follow "pre-requisite requirement" of "check[ing] oxygen analyzer for zero and span and log it in E/R log book." Nor did vessel follow SMS procedure "Vapor Balancing Planning" in that vessel did not "inject a span gas of known concentration for calibration and testing of the oxygen analyzer." Vessel used expired nitrogen span gas to calibrate the IG system on 5/28/2022 and also vessel did not log it as required.

Subject to regulation 3 of this Annex, the operation of a marine diesel engine that is installed on a ship constructed on or after 1 January 2016: is subject to the standards set forth in paragraph 5.1.1 of this regulation when the ship is operating in an emission control area designated under paragraph 6 of this regulation. On 06 May 2020, the Owner's Representative of the subject vessel reported a leak on the Main Engine Exhaust Gas re-circulation (EGR) cooler,rendering the EGR system inoperable. The cooler was blanked and the system isolated. As a result, the vessel's Main Engine would temporarily be non-compliant with MARPOL Annex VI, Reg. 13.5.1.1 (i.e. Tier III), which is required when the vessel is operating in an ECA area.
