PSCO observed flammable liquids stored in the Deck 5 Carpenter workshops (Cat. 13), which is prohibited without approved storage cabinets.

Fire doors in main vertical zone bulkheads, galley boundaries and stairway enclosures, the approximate time of closure for hinged fire doors shall be no more than 40 s and no less than 10 s from the beginning of their movement. PSCO observed multiple double leaf doors throughout the vessel failed to properly close, thus not meeting the time requirements provided above. FD A60 4.12, FD A0 4.14, and FD A60 5.10.

Doors required to be self closing shall not be fitted with hold back hooks. Several A and B class doors through the vessel had hold back devices to include hooks, wedges and tied with line preventing the door from self closing. 02 doors to launderette spaces had magnetic holdbacks that needed to be manually released. Additionally the fire door leading from the forward mooring area had a large carpet installed which caught, preventing the door form closing.

Passenger ships carrying more than 36 passengers shall be equipped with an automatic sprinkler system of an approved type complying with the requirements of the FSS Code in all control stations, accomodation and service spaces, including corridors and stariways. A guage indicating the pressure in the system shall be provided at each section stop valve. Guage at section valve A08-3 was not indicating the correct pressure.
