Cargo tanks shall be fitted with a visual and audible high-level alarm which indicates when the liquid level in the cargo tank approaches the normal full condition. Vessel has loaded past set 95% level in cargo hold #1, #3, and #6. PSCO observed the mentioned cargo hold level indicators for alarms not properly operating to set off high level alarms.
