"All arrangements used for pilot transfer shall efficiently fulfill their purpose of enabling pilots to embark and disembark safely." The vessel's pilot transfer arrangement was found to have stanchions on the landing platform that were not locked securely in place by safety pins.

In addition to the requirements contained elsewhere in the present regulations, ships shall be designed, constructed and maintained in compliance with structural, mechanical, and electrical requirements of a classification society which is recognized by the Administration which provide an equivalent level of safety. The main engine exhaust shied on the #2 and #3 turbo just aft of the heat sensor and before the turbo is missing the clips and is held on with safety wire allowing for heat to escape exposing the heat sensor to excessive heat causing discoloration and thermal breakdown. 74SOLAS(14)11-1/A-1.3-1
