The arrangement on board ship for the extraction of samples from the 15 ppm Bilge Separator discharge line to the 15 ppm Bilge Alarm should give a truly representative sample of the effluent with an adequate pressure and flow. PSCO observed that the OCM lacked any flow of effluent even after crew was given an opportunity to attempt repairs.

The machinery, shall be so installed and protected as to reduce to a minimum any danger to persons on board, due regard being paid to moving parts. PSCO observed that the #1 and #2 Ballast pumps and several other misc. pumps in the engine room were missing guards around their shafts.
