15113: The company should establish procedures to ensure that the ship is maintained in conformity with the provisions of the relevant rules and regulations and with any additional requirements which may be established by the company. During a marine casualty investigation to a loss of propulsion it was discovered that the solenoid valve (201) that controls the starting air valve the vessel became loose and prevented the engine from starting. Vessel could not provide organizational policy or procedures from the vessels operator on how to reinstall the solenoid valve to further prevent this from reoccuring.An internal (Class) audit is required to the satisfaction of the Coast Guard within 90 days.
